Summary

The PACT Care Coordinator is a Registered Nurse (RN) who will serve as a champion for patients and coordinates local implementation, training, communication, outreach and sustainability of PACT concepts and the principles of the PACT team. The nurse works in conjunction with the PACT team to ensure comprehensive, continuity, and continuation of care.The RN is responsible for coordinating and participating in huddles to ensure continuity of care within the PACT teams.

Responsibilities

The Registered Nurse (RN) is directly responsible to the nurse manager and associate chief nurse primary care. The RN Care Coordinator works in partnership with PACT team members to ensure comprehensive care. The RN uses the nursing process and evidence-based practice to collaborate with the team to include the provider and Licensed Practical Nurse (LPN) along with the PACT teams (Primary Care Provider, LPN, and Clinical Support Technician) and larger Team (family/caregiver, internal and community-based services involved in providing care to the patient) in developing the patient-driven holistic care plan for life.

Duties include, but not limited to:
  • Review consults with the provider to assess and anticipate what will be needed to arrange an initial consultative visit most efficiently.
  • Coordinate with PACT teams and resources such as social worker, medical support assistant, and other relevant professionals to facilitate visits.
  • Contact, assess, and coordinate care for Veterans who have had recent procedures to assure successful discharge and recovery from clinic visits or inpatient stays. Patient contact is to happen within two business days of discharge
  • Assess needs of established veterans, offer advice, and coordinate interventions with the provider.
  • Coordinate preparation and delivery of reports to Veterans and primary care providers.
  • Collaborate with the team LPN, MAS, and provider to assure smooth flow of patients through the clinic.
  • Communicate and interact appropriately and courteously with all internal and external customers
  • Maintain confidentiality of electronic, written, and/or verbal patient/employee information
  • Demonstrate working practices that include adherence to Infection Control standards and the safe use and operation of equipment
  • Acts as patient advocate
  • Performs other related duties as assigned.

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing ( ACEN ) or The Commission on Collegiate Nursing Education ( CCNE ). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

Preferred Experience: A minimum of two (2) years of recent, full time clinical experience as a RN in an PACT setting preferred.
    Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; ORa Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; ORADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.

Physical Requirements: The physical requirements of this position include visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required up to 45 pounds. The incumbent may be exposed to infected patients and contaminated materials and may be required to use personal protective clothing and/or equipment in isolation situations or operative/invasive procedures to prevent exposure to disease, illness and hazardous materials. The incumbent may be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.

BLS/ACLS Requirements: Current certification in Basis Life Support (BLS) and Advanced Cardiac Life Support (ACLS) is required for all staff providing direct patient care and working in MSICU. If you are selected, you will be required to have proof of certification prior to being boarded for appointment with VA or obtain such certification in order to avoid any delays in the hiring process.

Additional information

Annual leave shall accrue for full-time Nurses, and Nurse Anesthetists at the rate of 8 hours for each full biweekly pay period.
