Adventist HealthCare seeks to hire an Intake Liaison for Post-Acute Care (PAC) Services who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As a PAC Intake Liaison you will:
• Be responsible for ensuring the Post-Acute Care services department receives its Home Health and Rehabilitation referral and admission goals within their assigned hospitals.
• The position partners and interfaces with Case Managers, Social Workers, and Physicians with referring facilities, in order to efficiently complete the transition to acute rehabilitation or home health.
• Achieve admission goals by developing and maintaining referral relationships, and securing admissions or readmissions to the home care agency.
• Hold expertise in educating patients and families on the benefits of AHC's Post-Acute Services and screening patients for eligibility for PAC services.

Qualifications include:
• Bachelor's Degree required
• Active Maryland license as Registered Nurse (RN) or Licensed Practical Nurse (LPN) required
• 3+ years clinical experience in hospital or post-acute setting
• Preferred experience as a Clinical Liaison or Case Manager
• Hold expertise in referral processes and electronic systems
• Self-motivated with excellent time management skills; and be able to work both collaboratively and independently.

Work Schedule:
