Responsibilities

D. Role Responsibilities and Accountabilities
The ARNP is directly responsible to the Chief, Cardiothoracic Surgery, with ultimate accountability to the Chief, Surgical Services. The ARNP functions relatively independently as guided by approved clinical protocols, and privileges, functioning within the scope of practice for a licensed ARNP. Ongoing collaboration with physicians, nursing staff and other health team members is essential. The ARNP is accountable for all decisions and actions in the care of assigned patients under the direct supervision of the assigned Physician Preceptor. The Preceptor will review medical records and clinical practice of the ARNP.

The ARNP is a registered nurse practitioner who has completed the necessary educational
preparation in order to: engage in primary health care decision making; obtain advanced
competencies and skills for assessing physical and psychosocial health/illness status of
individuals, families and groups; promote health maintenance and protection against
disease; and, manage the care of patients/clients within the scope of preparation and
accountability.

The ARNP provides care within a defined scope of practice. Duties/tasks include, but are not be limited to: performing initial and interim history and physical examinations; ordering laboratory, radiologic and other diagnostic tests and providing initial interpretations; ordering EKG's and providing initial interpretation; screening patients for disease prevention and identifying health risks and needs; formulating a diagnosis based on history, physical examination and clinical findings; developing and implementing a treatment plan, ordering appropriate pharmacological agents, referring to other healthcare professionals or community agencies as appropriate; assessing patient's learning needs, developing, implementing plans to meet needs, and instructing patients and families in self-care and treatment; documenting of assessments, reassessments, differential diagnosis, plan of care and patient education in the clinical record.

The ARNP will function as an integral member of the Heart Transplant Team, working with advanced heart failure providers, transplant coordinator, cardiac surgery team, social workers, and physical therapists.

The ARNP is also responsible and accountable for maintaining BLS (and ACLS training when required) and for demonstrating the educational, experiential and knowledge/practice-based competency requirements outlined in the four (4) dimensions:

DIMENSION 1: CLINICAL
  • Provides a full range of nursing care to cardiothoracic patients with complex care needs, including assessments/reassessments and client condition and response to treatment and other clinical findings, assuming responsibility and accountability for nursing actions.
  • Assesses, develops and implements a plan to meet client's learning needs, including self-care and treatment measures. Modifies approach based on knowledge of human behavior, motivations and reactions to situations.
  • Engages co-workers/team in active problem-solving regarding clinical issues leading to an improvement in care.
  • Coordinates all aspects of care of patients with ventricular assist device patients in consultation with physicians, mid-level providers, transplant team members, and rehabilitative therapists.
  • Coordinates long term management and follow-up care of outpatients' post-transplant or implant.
  • Ensures optimization of patient's clinical, functional and psychosocial status with attention to the details of all aspects of care.
  • Demonstrates excellence in writing and computer skills, and accurately documents in the medical record, patient findings, assessments, and care provided.
  • Functions effectively in a leadership role that guides and directs others who provide patient care.
  • Applies a collaborative team approach in identifying, analyzing and resolving problems.
  • Participates in daily rounds with Cardiothoracic Surgery team on all inpatients.
  • Coordinates patient follow-up visits for both routine and emergent issues.
  • Maintains continuous communication with community agencies and patient's home VAMC regarding ongoing care and support of cardiothoracic surgical patients.
  • Provides expert clinical and technical support to medical, nursing, and other health care staff in operation of ventricular assist devices, including emergent evaluation of problems, development, and implementation of solutions.
  • Assists with pump assembly and preparation and intra-operative device management during VAD implants.
  • Assists with explant of pump during all heart transplant surgeries.
  • Works effectively and efficiently with high volume workload, multiple tasks requiring immediate attention, and multiple deadlines that must be met in a timely manner.

    Qualifications

    Basic Required Qualifications (Per Handbook 5005) Nurse Practitioner
    • U.S. Citizenship, Birth or Naturalized (appropriate documentation required)
    • Master's degree from an NLN or CCNE accredited Nurse Practitioner program
    • National Certification as a Nurse Practitioner
    • Current, full, unrestricted license as a Registered Nurse
    • State approval to practice as an NP
    • At least one-year experience in CT Surgical Service managing one or more surgical specialty sections
    Must be proficient in spoken and written English as required by 38 USC 7402(d), 7407(d). Preferred Qualifications
    • Minimum of at least two years of successful practice as a Nurse Practitioner providing medical care to patients
    • Demonstrated ability to work effectively with other disciplines
    • Demonstrated excellence in written/verbal communication and time management
    • Demonstrated ability to use computer systems in support of patient care delivery
    • At least two-years' experience with Surgical Service with Cardiothoracic/Transplant/Ventricle Assist Devices Strongly Preferred.
    Preferred Level of Education Nurse Practitioner
    MSN with Adult, Family or Primary NP or DNP

    Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:

    · Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
    · Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
    · Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
    · Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
    · Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.

    Physical Requirements: The position includes physical exertion such as regular and recurring walking, stooping, bending, reaching and lifting of equipment and supplies, and hand-eye coordination to perform visual field evaluations. It may require frequent periods of walking throughout the facility to interface with other services, bending, stooping, lifting, and carrying. The incumbent is exposed to CRT/PC screen radiation sometimes for long periods of time.     Annual leave shall accrue for full-time Nurses, and Nurse Anesthetists at the rate of 8 hours for each full biweekly pay period.
