Adventist HealthCare seeks to hire an experienced Clinical Practice Transformation Consultant who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

The Clinical Practice Transformation Consultant will be responsible for developing a productive relationship with the One Health Quality practicing practices to achieve Adventist Healthcare's strategic goals. The Consultant will provide technical assistance to the practices, serving as a key resource in practice's transformation.

As a Clinical Practice Transformation Consultant, you will:
• Develop strong working relationships, trust, and reliability with One Health Quality Alliance (OHQA) CIN and Care Transformation Organization (CTO) participating practices to achieve desired outcomes
• Develop plans and strategy in collaboration with OHQA and CTO practices
Communicate weekly progress and measurable results to leadership and other stakeholders and in data validation
• Assist in communicating key data trends for the purposes of strategy in managing risk stratification patient populations and quality metric gaps
• Work directly with physician offices to assist them with defined project support services to include MDPCP reporting, CAHPS roster, and clinical quality metrics submission.
• Contribute and lead practice transformation improvement projects
• Participate in OHQA and CTO Board and its subcommittees and other required meetings

Qualifications include:
• Bachelor's Degree required, MBA or MHA preferred
• 2-3 years of experience required, preferably in a healthcare setting
• Work experience in a physician practice is preferred
• Ability to work in high volume environment.

Work Schedule:

Full-time days, M-F, 8:30 am to 5:00 pm.
