Job Description St. Ambrose Housing Aid Center 321 East 25th Street Baltimore, MD 21218 (410)366-8550 Job Description and Announcement Case Manager Company Overview: The mission of St. Ambrose Housing Aid Center is to create and maintain equal housing opportunities for low and moderate-income people, primarily in Baltimore City, and to encourage and support strong and diverse neighborhoods. Job Summary: St. Ambrose Housing Aid Center seeks a full-time (35 hour/week) Housing Upgrades to Benefit Seniors (HUBS) Case Manager. The HUBS program serves residents of Baltimore City. The Case Manager will provide case management, outreach, and support services for older adults in Baltimore City. The case manager is responsible for conducting client assessments, assisting clients with meeting requirements to access grant and loan funding for home repairs & modifications, maintaining accurate records on all ongoing cases, completing monthly, quarterly, and annual reporting, and attending meetings and outreach events for the program. The Case Manager should be self-motivated, interested in assisting older adults, and interested in promoting and supporting the Central Baltimore community. The Case Manager is responsible for developing specific strategies to expand the program. Identify, contact, and assist a minimum of 75 low-income older adults per year with issues such as home repair, home modification, or weatherization services;? ? Connect older adults to other housing services, which include handyman services, tax credits; foreclosure prevention, and tax sale prevention through the completion of city, state, and nonprofit applications;? ? Assist older adults in applying for resources or benefits that make their homes safer and more affordable;? ? Provide quarterly and annual program, and financial data reports to management as required;? ? Maintain records and report activities to funding sources;? ? Assist with budget preparation and monitoring for programs;? ? Facilitate community service projects that involve and support older adults;? ? Collaborate with community, city, state, federal, and other social service agencies to support program goals.? ? Create cohesive programming to achieve the projects selected by program participants and aligns with neighborhood goals as appropriate;? ? Attend meetings as assigned by St. Ambrose Housing;? ? Advocate with agencies or persons to help clients receive appropriate benefits or services;? ? Identify and recruit volunteers;? ? Expand existing programs and identify additional resources;? ? Uphold high organizational standards. Performs related duties or responsibilities as assigned or requested.? Skills and Abilities: Case Manager Page 2 ? Bachelor's degree in social work, human services, or gerontology, or equivalent combination of education and work experience? ? Sensitivity in working with older adults;? ? Experience conducting needs assessments and initiating support services;? ? Must have practical interpersonal skills with experience working with diverse populations;? ? Strong written and oral communication skills;? ? Must have excellent problem-solving skills: Strong computer skills;? ? Must have the ability to motivate and connect with others successfully;? ? Must be highly organized and self-motivated with practical time management skills;? ? Ability to understand and follow complex written and oral instructions, rules, and procedures; to work independently and to be a vital and contributing part of a team;? ? Ability to effectively handle associated administrative responsibilities; including dealing with funder requirements, maintaining records, and meeting reporting requirements;? ? Ability and commitment to handle privileged information professionally and confidentially;? ? Must have dependable, insured transportation and a valid Maryland Driver's license and acceptable driving record. A DMV printout and proof of insurance will be required. Willingness to drive throughout Baltimore City.?
