For a view into our world, please visit our website at www.birminghamgreen.org MDS Coordinator Summary: Coordinates resident care planning activities, including the MDS and care plan process. Requires frequent interaction with families and staff. Responsible for timeliness of resident assessments. Will also perform other duties as assigned. Required Knowledge, Skills and Abilities: Skilled Nursing Facility (SNF) Medicare Patient-Driven Payment Model (PDPM) Medicare Billing and Reimbursement Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports. Ability to work as a team member with staff of Birmingham Green. Understanding of the behavior and needs of elderly people. Ability to communicate effectively to the residents, families, visitors and staff. Accuracy in documentation is essential. Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several variables. Excellent organization skills and accuracy in following directions, observing, interpreting, and recording essential. Education and Experience: Registered Nurse Graduated from an accredited school of nursing. Current Virginia RN License, CPR, MDS Certification. Working Knowledge of MDS and Care Plan process. Basic computer knowledge. Excellent communication and interpersonal skills. Essential Functions: . Care Plan Process . Coordinates and leads care plan meetings: schedules conferences and communicates schedules to appropriate staff members, residents, and family members and/or responsible parties. . Assures all facility disciplines necessary are present at and participate in Care Plan meetings including CNAs. . Communicates all care plan team recommendations to the appropriate disciplines. . Assists with daily upkeep of care plans. . Serves as resource person for resident care planning activities. Responsibilities | Insures accuracy of MDS assessments, including admission, annual, quarterly, significant change assessments, and all required Medicare PPS assessments, as well as the RAPS where indicated. | Gathers resident data and consults with service providers to identify actual and potential resident care needs including potential for Medicare. | Conducts weekly Medicare meetings. Instructs nursing staff on documentation needs for Medicare requirements. Reviews charts to assure proper documentation being performed. | Determines Medicare discharge dates after consulting with all disciplines. | Reviews ongoing resident weights and identifies any significant change in weight status. | Reviews entire MDS report for completeness and communicates to other disciplines areas of deficient information. | Inputs information into computer for electronic transmission.
