Shady Grove Medical seeks to hire an experienced Employee Health Plan Care Navigator who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As the The Employee Health Plan Care Navigator, you will be:
•Responsible for identification and coordination of medical care needs for the Adventist Health Net (employee) population.
•Completing assessments, planning, implementation, and coordination of care of high-cost / high-risk members of the health plan to provide optimal patient outcomes.
•Encouraging care within the Adventist HealthCare network when possible.
•Utilizing data and analytics to identify high cost, high risk, rising cost, rising risk individuals who could benefit from Care Navigation
•Working with enrolled individuals and their care team to develop a customized plan of care to return to optimal health. Navigate short-term, mid-term, and long-term cases
•Assisting enrolled individuals to follow the plan of care including coordinating appointments, collecting medical records, and other support as indicated
•Communicating with Utilization Management staff about any delays or barriers to care

Qualifications Include:
•Bachelor of Science in Nursing preferred, but not required
•Minimum of 3 years of clinical experience required
•Prior case management or discharge planning experience is preferred
•Active RN license is required

Those testing positive are given the opportunity to re-apply in 90 days, if they can truthfully attest that they have not used any nicotine products in the past ninety (90) days and successfully pass follow-up testing.
