Summary

The incumbent functions at the highest level of clinical practice, working independently under his/her scope of practice as defined by the medical center to directly care for patients requiring care as deemed necessary and appropriate in his/her designated area of specialty at the parent facility and to other areas of the medical center as assigned. This Clinical Program Manager is responsible for the professional and administrative management of an assigned area in pharmacy service.

Responsibilities

Major duties include, but are not limited to, the following:

SUPERVISORY FUNCTIONS
  • Manages the activities of assigned Clinical Pharmacy Specialist sand has the oversight of ongoing clinical pharmacy services. Works under the supervision of the Associate Chief of Pharmacy. Works with other pharmacy staff to assure that pharmacy clinical services are well coordinated and incorporated into the overall approach to patient care. Assures that the pharmaceutical care needs of patients are met across the continuum of care within the scope of their clinical specialty area.
  • The incumbent has full administrative authority over subordinates in his/her area of specialty, and ensures that services provided meet or exceed the professional and legal standard. Manages people, maintains effective interdepartmental relations, and cooperates with other services to accomplish the medical facility's mission and goals.
  • Ensures implementation of performance standards, assures that clinical practice is compliant with all regulatory and accrediting body requirements. Develops policies and procedures relative to the assigned area of practice.
  • Designs and implements orientation and training programs for staff, and develops and maintains systems to monitor the performance of staff activities.
  • Interviews prospective candidates for appointments, recommends appointments, advancements, or promotions or disciplinary actions. Incumbent will have direct or indirect oversight of the Pharmacy Residency Program and other educational programs if applicable to his/her area of practice. S/he may serve as a Residency Program Director (RPD) if a residency program is established at their site and if selected for the position of RPD. The incumbent will operate under a broad delegated authority from the medical center.
  • Supports the Chief of Pharmacy as needed during various inspections and surveys such as Joint Commission.
CLINICAL FUNCTIONS
  • Coordinates all clinical pharmacy functions for his/her assigned practice area, helping to achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams. Attends and participates in team meetings, conferences, and other scheduled events as required.
  • Designs, implements, assesses, monitors and documents therapeutic plans utilizing the most effective, least toxic, and most economical medication treatment for patients consulted to his/her care and other clinics as designated by supervisor.
  • Works independently and effectively to evaluate prescriptions and/or provider orders for accuracy, eligibility, availability, and cost effectiveness and therapeutic appropriateness; contacts physicians and makes authorized changes in orders or prescriptions as required.
  • Functions independently in day-to-day clinical and administrative duties associated with the position, consulting with higher level supervisors only when required. In general, uses a high degree of appropriate professional judgment to solve complex problems which routinely arise throughout the work day. When required, communicates any problems to the medical staff and/or other health care professionals, and offers alternatives or solutions to the problems.
  • Demonstrates recognized expertise in the assigned area of practice for the purpose of management of several disease states specific to his/her area of clinical specialty. These specialties include-but not limited to - diabetes, hypertension, hyperlipidemia, anticoagulation, COPD, and heart failure-issues.
  • Monitors and assesses the outcome of drug therapies with advanced skill, including physical assessment and interpretation of laboratory and other diagnostic procedures. Monitors patients prospectively for any deviation from the accepted treatment plan, for the efficacy of the plan, and for any adverse reaction or drug interaction occurring during the execution of the plan.
  • Demonstrates advanced knowledge of pharmaceutics, pharmacodynamics and pharmacotherapeutics as determined by peer review. Reviews, analyzes, and applies literature to specific patient situations.
  • Documents interventions and recommendations via CPRS progress notes and consults, demonstrating the ability to communicate orally and in writing to persuade and influence clinical and management decisions. Written documentation of all patient interactions is required.
  • Demonstrates knowledge of the standards related to Ambulatory Care, Hospital, and Behavioral Health drug use. This includes, but is not limited to, the standards established by DEA, FDA, VA, the state, and The Joint Commission.

Work Schedule: Monday-Friday 7:45am-4:30pm
Telework: This a position is " telework eligible" ; however, it is contingent to meeting all criteria, qualification, and completing all training.

BASIC REQUIREMENTS

U.S. citizenship.

English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).

Education.
(1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy). TRANSCRIPTS MUST BE SUBMITTED WITH APPLICATION

(2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. Creditable Experience. Knowledge of Professional Pharmacy Practices. To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice. Professional practice means paid/non-paid employment as a professional or unlicensed graduate pharmacist as defined by the appropriate licensing board.

Residency and Fellowship Training. Residency and fellowship training programs in a specialized area of clinical pharmacy practice may be substituted for creditable experience on a year-for-year basis. The pharmacy residency program must be accredited by the American Society of Health-System Pharmacists (ASHP). A fellowship program that is not accredited by the American College of Clinical Pharmacy (ACCP) will need to have comparable standards for experience to be creditable (Professional Standards Board refers to the Deputy Chief Consultant for Professional Practice for the determination).

Quality of Experience. Qualifying experience must be at a level comparable to pharmacy experience at the next lower level. Experience as a Graduate Pharmacist is creditable provided the candidate was used as a professional pharmacist (under supervision) and subsequently passed the appropriate licensure examination.

Part-time Experience. Part-time experience as a professional pharmacist is credited according to its relationship to the full-time workweek. For example, a pharmacist employed 20 hours a week, or on a 1/2-time basis, would receive 1 full-time workweek of credit for each 2 weeks of service.

Preferred Experience: Must have completed a PGY1 pharmacy residency or have equivalent experience.

Grade Determinations: In addition to the basic requirements for employment stated above, the following criteria must be met when determining the grade of candidates.

(a) Experience or Education. In addition to the basic requirements, candidates must meet one of the following:

1. One year of experience equivalent to the next lower grade level (GS12)

or

2. Completion of an ACPE-accredited Pharm.D. program.

(b) Assignments. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. This position reports directly to the Associate Chief, Pharmacy, Primary Care Services.Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs):
  • Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
  • Expert understanding of regulatory and quality standards pertaining for their program area.
  • Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
  • Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
  • Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

    The full performance level of this vacancy is GS-13.

    Physical Requirements: This position requires visual acuity (depth perception and ability to distinguish basic colors and shades of colors), near vision correctable at 13 to 16, and far vision correctable in one eye to 20/20 and 20/40 in the other and keen hearing. This position requires potentially long periods standing (6 hours). The incumbent may be required to do light lifting and carrying of up to 15 pounds; reaching above shoulders. Both hands and legs are required along with the use of all fingers; hearing (aid permitted); repetitive computer use.
