Shady Grove Medical Center seeks to hire an experienced Care Navigator , Behavioral Health Community Health Worker who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As a Care Navigator, Behavioral Health Community Worker you will:
•Collaborate with the Behavioral Health Care Navigator, completes comprehensive psychosocial assessment with focus on patient's requirements as they transition to the next level of care.
•Collaborats with Care Navigator and interdisciplinary team to foster a coordinated approach to discharge planning.
•At the direction of the Care Navigator, communicate with Utilization Review staff about any denials, issues or barriers to discharge. Obtains authorizations for needed outpatient services.
•Identify services and resources available in the community and assists with patient connection to these services
•Assist patients to schedule medical appointments to include connection with transportation

Qualifications Include:
•Bachelor of Science in health education, health promotion, or other related health field.
•2 years of healthcare experience (behavioral health experience preferred)
•Community Health Worker certification preferred
•Licensed driver and able to operate a motor vehicle as well as maintain automobile insurance as directed by AHC
