Develops, promotes and evaluates community-based programs for older adults with disabilities that are supported through the Community Health LTC Program Development Unit. Provides guidance to advance development of aging-in-place Villages and give on-going support to existing Villages. Conducts community outreach activities to promote programs and initiatives to neighborhood associations, educational institutions, professionals associations, and other community based organizations. Provides consultation and assistance in the selection, development, utilization and evaluation of LTC projects. Provides project management and coordination activities for committees, task forces or teams to advance community LTC program development. Conducts statistical analysis, establish controls to monitor quality of delivery of designated program(s) and services; research and identify funding sources and develop grant/funding applications.

The functional areas of this position are: developing programs and partnerships, community engagement, coalition building, and data analysis.

Illustrative Duties

Performs a wide-range of professional work in multiple administration functions within an organizational unit including analysis and project management;
May supervise administrative and paraprofessional staff;
Performs a variety of technical and professional work related to monitoring and reporting on departmental business processes, activities, and outcomes;
Plans, develops, and implements comprehensive communication and education plans on programs and services for both county agencies and community organizations;
Assists with developing and conducting special studies and research initiatives, by designing survey instruments, gathering data and information from department staff, developing data analysis tools, conducting statistical analyses, initiating literature reviews, etc.;
Conducts analysis and reporting of quantitative and qualitative data in order to track and monitor various business process indicators;
Serves as the point-person and liaison within the department and with central county
departments regarding a designated program and functional areas of responsibility;
Prepares reports and presentations of analysis and findings.

Required Knowledge Skills and Abilities

Knowledge of the mission, goals and objectives of the organizational unit, program or activities to which incumbent is assigned;
Knowledge of the principles, practices, and techniques relating to various functional areas of business operation (e.g., personnel, budgeting and financial management, contract administration and management);
Knowledge of the principles, practices and techniques governing management, organization, operations and programmatic services;
Ability to identify and describe a range of possible solutions for solving business problems;
Ability to apply research methods to design studies and assessments, and statistical analysis techniques to identify patterns and trends in data;
Ability to use word processing and presentation software to prepare documents, and to use spreadsheet and statistical analysis software packages to store, manipulate, analyze and present data;
Ability to train, lead, and/or supervise paraprofessional staff.

Employment Standards

MINIMUM QUALIFICATIONS:
Any combination of education, experience, and training equivalent to the following:

Graduation from an accredited four-year college or university with a bachelor's degree in the field related to the assigned functional area; plus two years of professional work experience within the functional area.

CERTIFICATES AND LICENSES REQUIRED:
Driver's license
Cardiopulmonary Resuscitation - CPR (Required within 60 days)
AED (Required within 60 days)

PREFERRED QUALIFICATIONS:
  • Bachelor's or master's degree in social work, public relations or community development.
  • At least two years community engagement experience.
  • Experience with and knowledge about aging in place Village programs preferred.
  • Knowledge about trends and needs of older adults at the local, state and national level.
  • Experience with developing and maintaining partnerships, community engagement and building coalitions; track record of team leadership with multiple stakeholders.
  • Proven ability to communicate (written and verbal), organizational and interpersonal skills.
  • Proven ability to develop outcome-focused data collection methods and analysis.
  • Possess strong critical thinking skills, demonstrate problem-solving skills, and the ability to exercise sound judgment.
  • Ability to manage multiple tasks simultaneously and work in a team environment and/or independently. Proficient in Microsoft Office Suite and other software applications.
  • Ability to plan and implement activities in order to ensure effective and efficient utilization of resources.
NECESSARY SPECIAL REQUIREMENTS:
The appointee to this position will be required to complete a criminal background check, a Child Protective Services Registry, and a driving record check to the satisfaction of the employer.
Note: This position is considered "Emergency Service Personnel"

PHYSICAL REQUIREMENTS:
Job is generally sedentary in nature, and job requires walking, standing, sitting (for long periods of time), kneeling, reaching, bending, climbing stairs; uses hands to grasp, handle, or feel. Visual acuity is required to read data on a computer monitor; ability to operate keyboard driven equipment and computer; ability to drive a motor vehicle. Position frequently communicates and must be able to exchange accurate information with others verbally and in writing. Employee may be required to lift or carry up to 25 lbs. occasionally. Generally works in an office environment yet may occasionally be required to perform job duties outside of the typical office setting. All duties performed with or without reasonable accommodations.
