Adventist Health Care seeks to hire an experienced RN or SW Case Manager who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As a Case Manager, you will:
•Provide case management and coordination of services to patients and families within the interdisciplinary system of acute in-patient rehabilitation.
•Manage the processes involved in rehabilitation; coordinates, integrates and educates patients/families and continuum of care based upon patient's individual medical, physical and psychological needs.

Qualifications Included:
•BSN/Master of Social Work from an accredited university
•Active Maryland RN/Maryland Social Worker license required (LCSW-C strongly preferred)
•Minimum three years prior experience within a hospital case management environment preferred
