Works as part of the Health Services Division of the Fairfax County Health Department and is guides professional and paraprofessional staff across multiple service sites to ensure the overall functioning, efficiency, and effectiveness of home visiting services provided by the Nurse-Family Partnership (NFP) program. Manages material and human resources and provides direction, guidance, and support for staff to ensure consistent delivery of NFP services. Must be able to work with minimal supervision and as a collaborative partner in a Maternal Child Health (MCH) management team. Responsibilities include coordinating referrals for a variety of home visiting services provided by PHN staff (Maternal Child Health Field, Nurse-Family Partnership, Healthy Families); monitoring and evaluating the quality and effectiveness of services provided; guiding and supervising professional and paraprofessional staff; prioritizing assignments and activities in accordance with program, agency goals and objectives; data collection, analysis, and reporting; representing the program on committees and work groups; and collaborating with colleagues and community partners to reach established goals and objectives.

Employment Standards

MINIMUM QUALIFICATIONS:
Graduation from a college or university accredited by the National League for Nursing or the Commission on Collegiate Nursing Education with a bachelor's degree in Nursing;
Plus five years of post-licensure experience as a nurse in an equivalent health care setting whose focus was primarily prevention services and community education, including three years in a public health/community health setting.
Qualifying experience may include experience in ambulatory care settings such as outpatient clinics that incorporate community outreach/education as part of services or specialty settings that incorporate teaching, community outreach, resource referrals, etc. as part of a health care continuum;
Plus at least one year of supervisory experience.

CERTIFICATES AND LICENSES REQUIRED:
  • Possession of current license or a multistate licensure privilege to practice as a Registered Nurse in the Commonwealth of Virginia
  • Valid motor vehicle driver's license
  • Cardiopulmonary Resuscitation - CPR (Required within 60 days)
  • AED (Required within 60 days)


NECESSARY SPECIAL REQUIREMENTS:
The appointee to this position will be required to complete a criminal background check, a Child Protective Services Registry, a driving record check, and sanction screening to the satisfaction of the employer.

This position is emergency/essential services personnel. (Required to report for duty during inclement weather and/or other emergencies).

PREFERRED QUALIFICATIONS:
  • Master's degree in nursing, public health, or nursing administration preferred.
  • At least two years of management and professional staff supervision experience.
  • Two or more years of post-licensure experience providing health services to pregnant and/or post-partum women, infants, and/or children, including at least one year of experience in case management and/or care coordination in a public health/community health setting.
  • Highly desirable to have the following professional experience: providing Nurse-Family Partnership services; maternal child home visiting; care coordination for high risk pregnant women and infants; working on multi-disciplinary teams; community partnership development; working with ethnically diverse populations; health education/health promotion, and data management and reporting.
  • Excellent oral, written, and interpersonal communication skills.


PHYSICAL REQUIREMENTS:
Ability to walk, stand, sit, bend, kneel, reach, climb stairs and carry equipment or young children up to 25 pounds in weight. Ability to use hands to grasp, handle or feel. Visual acuity to read data on a computer monitor. Ability to operate keyboard driven equipment and drive a vehicle. Ability frequently communicate and exchange accurate information with others verbally and in writing. All duties performed with or without reasonable accommodations.
