Responsibilities include care transitions across the system's healthcare continuum, to include the various settings in the hospital, ambulatory and post- acute areas. Alignment with system goals, resources, educational, CMS, State and DNV regulatory requirements and strategic opportunities for the system in collaboration with leadership and members of the multidisciplinary healthcare team members. Accountable for implementing all contractual agreements and requirements, compliance with Federal, State and Accreditation Standards for Care Transitions. Areas of responsibility include both strategic planning and support in the following areas: medical and healthcare guidelines, quality and patient safety initiatives, new technologies, criteria compliance, transitions of care and utilization management, and coding compliance.
