Our Post Acute Care Center seeks to hire an experienced PAC Services Assistant for our Post Acute Care Services Department who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As a PAC Services Assistant you will:
• Start/support the referral process for Post-Acute patients utilizing a variety of resources
and systems.
• Perform clerical duties to support the PAC Center team, including (but not limited to) tracking
re-hospitalized patients, capturing resumptions of care, and monitoring electronic systems
for new referrals.
• Provide reports and other support to management as requested.

Qualifications include:
• High School diploma or equivalent required
• AA Degree in Business or Health Care preferred
• 1-2 years Home Care, Acute Rehab, Sub Acute Rehab, or other Post-Acute setting preferred

Work Schedule:

Full-time 40 hours/week during daytime hours.

Schedule includes Saturdays and Sundays + 3 other weekdays.
