Adventist Health Care seeks to hire an experienced Inpatient Care Navigator, who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As the IP Care Navigator, you will:
• Identify and coordinate patient care needs in anticipation of the next site of care after hospitalization. This involves timely assessment, planning, implementation and coordination of care to provide optimal patient outcomes. Provides guidance for clinical coordination, and collaboration among multidisciplinary caregivers.
•Complete comprehensive psychosocial assessment and review of clinical needs of patients
with focus on patient's requirements as they transition to the next level of care
•Identify and navigates patient testing and treatment to reduce barriers to patient discharge and preventing delays in patient care

Qualifications Include:
• Graduate of an accredited school of nursing with a minimum of a Bachelor's Degree.
• Active Maryland RN license required
• Active American Heart Association Basic Life Support (BLS) certification required
• At least 5 years of recent case management experience in a hospital setting and utilization review experience.
