Adventist Health Care seeks to hire an experienced Community Health Worker who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As the Community Health Worker, you will:
•Help patients and their families to navigate and access community resources. These resources include access to medical and/or behavioral health providers, medication, food, transportation, housing, and insurance. In collaboration with the RN Patient Navigator, the Community Health Worker guides patients through their plan of care.
•Coordinate patient care with patient, family, and ancillary/community services to promote quality and cost-efficient care for assigned caseload.
•Provide follow-up to patients, by phone, home visits and visits to other settings where patients can be found
•Conduct home visit to ensure that the patient has what is required to recover safely at home; visit to include safety checks (e.g., check for fall risks) and preparation for follow-up appointments with provider

Qualifications Include:
•Knowledge of community resources
•Bachelor of Science in health education, health promotion, related field or relevant work experience
•3+ years' experience working in a community-based setting with knowledge of local resources and system navigation
•Certification as Community Health Worker is preferred
•Licensed driver and able to operate a motor vehicle as well as maintain automobile insurance
