Job Announcement

The Fairfax County Health Department (FCHD) is a progressive public health leader providing services that promote population health, protect public health and the environment, and ensure residents' equitable access to health services and information. Works directly with clients enrolled in the Chronic Disease Care Coordination program to support the client in effective management of his/her chronic health conditions, self-care, linking with community resources and adopting healthy behaviors. Establishes trusting relationships with clients/caregivers and families while providing general support and encouragement. Provides these services in the client’s home, community and clinic settings. Collaborates with the health care team, providers and community partners in implementation of the client’s individualized health care plan. Provides social support, informal counseling, and advocates for individuals and community health needs, and provides services such as home visits, small group education and community outreach. Works under the direction of a Public Health Nurse (PHN).

These are 30 hour/week merit positions.


Required Knowledge Skills and Abilities

Knowledge of the basic principles of human behavior;
Knowledge of word processing and data management programs (Excel, survey software)
Ability to communicate effectively to clients;
Ability to organize information and records;
Ability to communicate in written format;

MINIMUM QUALIFICATIONS:

Any combination of education, experience and training equivalent to graduation from high school or GED issued by state department of education; plus three years of work experience, including two years in some area of human services (related two-year [associate] degree may be substituted for the required human service experience).

PREFERRED QUALIFICATIONS:
Preferred candidates will have knowledge and experience with care-coordination and/or health coaching. Ability to reach out and maintain positive community relationships. Possess exceptional Spanish/English verbal and written communication skills; candidates must pass language proficiency exam. Must have the ability to establish and maintain effective working relationships; work well with multi-disciplinary team members and community partners. Ability to exercise tact, discretion, initiative and independent judgment. Excellent customer service skills with the ability to interact in a friendly, service-oriented and professional manner.
CERTIFICATES AND LICENSES REQUIRED:
Driver's license
AED (Required within 60 days)
CPR (Required within 60 days)

NECESSARY SPECIAL REQUIREMENTS:
The appointee to the position must satisfactorily complete a criminal background check, a child protective services check, a sanction screening, a driving record check and tuberculosis test.

PHYSICAL REQUIREMENTS:
Employee is required to walk, stand, sit, bend, kneel, reach, climb stairs and carry equipment or young children up to 25 pounds in weight. Uses hands to grasp, handle or feel. Visual acuity is required to read data on a computer monitor. Ability to operate keyboard driven equipment. Ability to drive a vehicle. Position frequently communicates and must be able to exchange accurate information with others verbally and in writing. All duties performed with or without reasonable accommodations.
