White Oak Medical Center seeks to hire an experienced Care Navigator, Inpatient SW NEX, who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As a Care Navigator, you will:
•Identify and coordinate patient care needs in anticipation of the next site of care after hospitalization. This involves timely assessment, planning, implementation and coordination of care to provide optimal patient outcomes.
•Provide guidance for clinical coordination, and collaboration among multidisciplinary caregivers.
•Complete comprehensive psycho social assessment with focus on patient's requirements as they transition to the next level of care
•Collaborate with physicians, nurses, and other disciplines involved with care of the patient to foster a coordinated approach to discharge planning

Qualifications Include:
•Bachelor of Social Work (BSW) or Master of Social Work MSW
•Licensure required include LBSW, LMSW, or LCSW
•Minimum of 3 years in diverse clinical setting required
•Prior case management or discharge planning experience is preferred
•Working knowledge of federal, state and local laws that govern health care
•Knowledge of community resources
•Prior experience with Cerner EMR and All Scripts is preferred
