Shady Grove Medical Center seeks to hire an experienced Inpatient Care Navigator who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

The Inpatient Care Navigator collaborates with the patient, physician, case manager, and other healthcare providers to support patients as they move throughout the healthcare system.

As a Care Navigator you will:
•Identify, assess, plan, implement, coordinate, monitor, and evaluate healthcare services to meet an individual patient's specific health care needs in a cost-effective manner. (may include but not limited to home health assessment, telehealth initiation, disease management education, coordinate of post-acute services)
•Ensure weekly contact with the patient (in person or via telephone) to progress towards care plan goals. Conduct patient home assessment visit when indicated

Qualifications Include:
•Bachelor of Science in Nursing, MD RN License
•1-3 years' experience in patient navigation, discharge planning, or care coordination
•Licensed driver and able to operate a motor vehicle as well as maintain automobile insurance

Work Schedule:
  • 16 hours per pay period
  • Every other weekend Sat/Sun
  • 8 hour shifts
