Summary

The Neurology Clinical Pharmacy Specialist will be responsible for the planning, implementing and operating advance clinical pharmacy services between Neurology and Pharmacy Services. Core activities outlined in the individual scope of practice include medication prescriptive authority, assessments, consults, laboratory and other test ordering abilities for the Neurology service. This may include but is not limited to comprehensive medication management for disease states.

Responsibilities

Examples of Clinical Care duties included but not limited to:
1.)Responsible for the management and development of a highly innovative
pharmaceutical care service for the Neurology /specialty program/clinic. The majority of time is spent in the provision of clinical pharmaceutical care for the management of patients seen in the
neurology / pain management clinic:
2.)Obtains medication history from each new patient and summarizes the significant findings, both
documented in the medical record and reported to the responsible physician when appropriate.
3.) Identifies all drug-induced problems that may be affecting the patient or contributing to a need
for hospitalization. This would include drug side effects, allergic reactions, toxicities, improper
dosage regimens, non-compliance, etc.
4.)Makes recommendations to the responsible physician for optimal management of drug-induced
problems either verbally or in writing.
5.)Evaluates the risk-benefit ratio of various therapeutic alternatives, develops a therapeutic
plan utilizing the most efficacious, least toxic and most economical pharmacological treatment
modalities available, and communicates these plans to the patients' physician.
6.)Possess a high level of independence designated by their Scope of Practice and will
be recognized by their peers as performing assignments that require an exceptional level of competence.
7.)Manage the medication programs of patients by electronically ordering appropriate medications,diagnostic studies, etc.
8.) Selects appropriate objective and subjective parameters necessary to monitor the pharmacologic
and toxic effects of the drugs being administered.
9.) Analyzes the patients' medication regimen for drug-drug interactions and determines the potential
and significance of each interaction.
10.) Analyzes the patients' medication regimen and laboratory test data for drug interference with
laboratory tests.
11.)Develops rational alternative therapeutic treatment plans in cases where the initially selected plan fails
to achieve the desired therapeutic result, problems occur with drug toxicity or other complications and
communicates this information to the patient's physician.
12.) When assessing or providing patient care, the employee will evaluate the impact of the age of the
patient as it relates to ability to understand directions, provide objective self-assessment or any other
medical milieu appropriate to the overall treatment plan.
13.) Conducts and oversees at least biannual medication utilizations evaluations as assigned through P&T
and DUR Subcommittee.
14.)Will serve as subject matter consultant on checking the review and approval of non-formulary requests.

Examples of Education duties included but not limited to:
1.)Develop and manage all pharmacy educational programs for
the Neurology / Specialty program. This will include (but not limited to) residency programs,
clerkships/internship, staff development and university affiliated programs.
2.)Educating patients on the proper use and care of their medications through the day-to-day
personal interactions with the patients, as assigned.
3.)Precepts the Pharmacy Practice Residents during rotations in this institution's Pharmacy Practice
Residency Program.
4.)Attends and actively participates in all clinical pharmacy activities, i.e., staff development,
residency conferences, journal club.
5.)Contributes regularly to the Pharmacy Service Newsletter, a monthly newsletter prepared by the
Section of Clinical Pharmacy and distributed to all physicians, pharmacists, nurses and dietitians.
It provides scientific and technical articles, as well as general drug information and pharmacy
policy.
6.)Prepares and presents educational lectures to a variety of disciplines in the area of Primary Care topics.
7.)Precepts pharmacy students from affiliated institutions for their clinical rotations.
8.)Provides formal education and training programs in the area of neurology / pharmacology,
therapeutics, pharmacokinetics and related subjects to physicians, nursing staff and other health care
professionals as appropriate.
Other duty functions included but not limited to are Program Management, Quality Management, Research and Automated Information System.

Qualifications

Basic Requirements:
    U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Education. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria) .
Grade Determinations:
Must have 1 year of experience equivalent to the next lower grade (GS-12). Examples of experience includes handling routine medication-related activities in accordance with local, and national policies, reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; and providing oversight of technical staff in all aspects of medication distribution. In addition, pharmacists must demonstrate the following knowledge, skills and abilities (KSAs):
a. Ability to communicate orally and in writing to persuade and influence clinical and management
decisions.
b. Expert understanding of regulatory and quality standards for their program area.
c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their
program area or area of clinical expertise.
d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical
assessment and interpretation of laboratory and other diagnostic parameters.

Physical Requirements: The work requires regular and recurring physical exertion, involving standing/sitting for prolonged periods of time, lifting/carrying of pharmaceutical supplies weighing up to 50lbs, and bending/stooping. The work may entail repetitive wrist movement while using various size syringes, vial cap removal, and typing.
