Summary

The Registered Nurse, Mental Health Intensive Case Manager position is assigned to Nursing Service at the VA Medical Center, Washington, D.C and provides community-based clinical case management for patients who have severe and persistent mental illness.

Responsibilities

Major duties and responsibilities include but are not limited to:
  • Responsible for providing case management for patients who have severe and persistent mental illness;
  • Responsible for assessing, planning, implementing, evaluating and documenting care to promise health and achievement of treatment goals;
  • Provides nursing care and services within accepted standards of practice to include conducting home visits, conducting individual, family and/or group counseling and education sessions;
  • Will assist patients and/or family with various skill development; conducting, coordinating and/or participating in preventative health education;
  • Responsible for developing and maintaining therapeutic relationships with patients, families and the interdisciplinary team and conducting crisis management and completing medication management;
  • Will work in collaboration with an interdisciplinary team in the development of a recovery, rehabilitative and function-focused plan of care;
  • Responsible for collaborating and coordinating care including services as an advocate on behalf of the patient to ensure comprehensive service delivery and as a liaison between the VA and other community resources;Responsible for accurately documenting medical record patient findings, assessments and patient care provided;
  • Assigns staff to provide care based on staff competencies and identifies priorities to ensure safe and effective patient care.

Work Schedule: Full Time, Monday-Friday, Flexible 8-hour tours of duty; 7:30AM-4:00PM or 8:00AM-4:30PM

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. [This applies to the past, current, and upcoming VA Health Professional Trainees (HPT) only. HPT is an individual receiving VA Health Professional Scholarship Program or appointed to temporary positions in one or more VHA medical facilities performing clinical or research training experiences to satisfy program or degree requirements. HPTs can be either VA-paid or Without Compensation (WOC)].

Basic Requirements:
    United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Graduation from School of Nursing: Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing ( ACEN ) or The Commission on Collegiate Nursing Education ( CCNE ). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. In these programs, students are given a certificate of professional nursing to sit for the NCLEX and should submit this certification to VA prior to appointment. Students do not earn a BSN, but receive a MSN degree upon completion of course work. A copy of the MSN transcript must be provided to VA. Bridge programs that confer a master's degree also fully meet the education requirement, even though a bachelor's degree is not awarded. OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. (UNOFFICIAL TRANSCRIPT AND/OR COPY OF CERTIFICATION REQUIRED: If you do not submit your transcript showing conferred major, degree and date and/or copy of certification, your application will NOT be reviewed.) Registration: Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. (COPY OF CERTIFICATION REQUIRED: If you do not submit a copy of the certification, your application will NOT be reviewed.) English Language Proficiency: RNs appointed to direct patient care positions must be proficient in spoken and written English as required by VA Policy
Preferred Experience: Case Management Experience is Preferred.

Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
  • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
  • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
  • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
  • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.

Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patient and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, and able to shift priorities based on patient needs.

Additional information

Annual leave shall accrue for full-time Nurses, and Nurse Anesthetists at the rate of 8 hours for each full biweekly pay period.
