White Oak Medical Center seeks to hire an experienced Care Navigator, Inpatient SW NEX, who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As a Care Navigator, you will:
•Responsible for identifying and coordinating patient care needs in anticipation of the next site of care after hospitalization. This involves timely assessment, planning, implementation and coordination of care to provide optimal patient outcomes.
•Provides guidance for clinical coordination, and collaboration among multidisciplinary caregivers.
•Completes comprehensive psycho social assessment with focus on patient's requirements as they transition to the next level of care
•Collaborates with physicians, nurses, and other disciplines involved with care of the patient to foster a coordinated approach to discharge planning
•Identifies and navigates patient testing and treatment to reduce barriers to patient discharge and preventing delays in patient care; communicates barriers to leadership for resolution and trending
•Participates in Interdisciplinary Rounds and other patient care conferences
•Documents assessments and interventions according to departmental standards

Qualifications Include:
•Effective use of time management skills to multitask, manage relationships with post-acute providers and gather resources for the department.
•Working knowledge of federal, state and local laws that govern health care and case management
•Master of Social Work (MSW) or Licensed Master Social Worker (LMSW)
•Minimum years of experience required, and what type of experience qualifies
•Minimum of 5 years social work experience in diverse clinical settings
•Licensed Social Worker for the State of Maryland
•Prior experience with Cerner EMR and All Scripts is preferred.
