Shady Grove Medical Center seeks to hire an experienced Inpatient Care Navigator, RN who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As the Inpatient Care Navigator, you will:
•Identify and coordinating patient care needs in anticipation of the next site of care after hospitalization
•Conduct timely assessment, planning, implementation and coordination of care to provide optimal patient outcomes
•Provide guidance for clinical coordination, and collaboration among multidisciplinary caregivers
•Complete comprehensive psycho social assessment and review of clinical needs of patients
with focus on patient's requirements as they transition to the next level of care
•Collaborate with physicians, social workers and other disciplines to foster a coordinated approach to discharge planning

Qualifications Include:
•BS in Nursing
•Minimum 5 years nursing experience required
•Maryland RN license
•Prior case management or discharge planning experience preferred
•Prior experience with Cerner and All Scripts preferred
