Adventist Health Care seeks to hire an experienced ED Case Manager, who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As the Turn Care Coordinator, you will:
•Work in the Emergency Department (ED) to identify, assess, and provide resources necessary to allow patients to manage their health outside of the ED.
•Support ED patients and staff to coordinate medical and social resources to promote safe and efficient discharge plans.
•Collaborate with patients, ED staff, ED physicians, and community providers to develop a comprehensive plan of care for safe discharge
•Act as a patient advocate by identifying and connecting patients to the most appropriate community resources. •Maintain a directory of available services by geographical area and update others about new services.
Consult with others to identify potential community resources for resolving client health, psychosocial, or financial problems.
•Review documentation on all potential admissions or those with orders for admission to ensure medical necessity is met and the correct level of care is assigned by utilizing a criterion database.

Qualifications Include:
• Bachelor's degree in Nursing OR Masters in SW
•Current MD RN licensure OR LGSW or LCSW or LCSW-C
•Bilingual preferred
•3-5 years' experience in a healthcare setting
•Strong assessment skills
•Ability to prioritize multiple tasks
•Ability to interact with various members of health care team

Work Schedule:

Monday-Friday

Hours are variable depending on needs of department

10am-10pm for 12 hour shifts

12pm-8pm / 2pm-10pm if 8 hour shifts
