Job Details

AHC Home Care Services seeks to hire an experienced Registered Nurse Preceptor (RN) who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As a Home Health Registered Nurse Preceptor (RN) you will:
• Orients and supervises clinical staff throughout the 90 day introductory/probationary period
• Serves as a mentor and clinical staff resource throughout the first 12 months hire for all new clinical employees
• Supports new clinicians throughout orientation using Orientation Checklist for OASIS accuracy, clinical documentation, clinical competency, home observation visits and electronic documentation support
• Performs performance improvement functions as needed to support advancing clinical education and knowledge that aligns with State, Federal and Accreditation requirements
• Conducts joint visits with field staff to assure clinical competence and OASIS Accuracy during orientation and on an ongoing basis as need is identified
• Plans and evaluates patient care that is comprehensive and organized to achieve relevant goals
• Documents patient care and related information according to Federal and State regulations, completed per standards set by Agency
• Manages an active caseload with varying volume
• Makes appropriate interventions to provide for safe and effective care addressing all of patient's needs
• Assists clinical staff in organizational skills to meet accuracy, efficiency and productivity standards
• Orients new staff to minimize low turnover rate during first 12 months of hire
• Advises staff on holistic approach to healthcare to ensure appropriate uses of MSW and Chaplain
• Supports accreditation process related to clinical competence and accountability

Qualifications include:
• Bachelor's degree in Nursing (BSN) preferred or completion within three years of hire into the role
• Current and Active Maryland RN licensure
• Current American Heart Association BLS/CPR Certification
• Two years full time experience in acute clinical, Medical/Surgical or facility nursing within the past five years of hire date preferred
• 1-year Home Health experience within the past two years
• 1-2 years supervisory/management experience preferred
• Valid driver's license and auto insurance that meets AHC required limits
• Successful completion of formal preceptor training program within the first 90 days of hire
