Job Announcement

The Fairfax County's Health and Human Services (HHS) organization is comprised of eight agencies and two community centers totaling approximately 4,000 staff with a total budget size of general, special, and grant fund support of $800 million dollars. The Office of Strategy Management (OSM) for HHS' mission is to execute and evaluate Health and Human Services policies, strategies, and priorities that ensure the efficient and effective allocation of resources under the leadership of the Deputy County Executive of HHS and Director of OSM.

The Health Integration Safety Net Director is a key member of the OSM leadership team and the entire HHS organization. As a recently established position, one will lead and manage the County's strategic development, advancement, and monitoring of a comprehensive and integrated health safety net system.
This will include the following responsibilities:
  • Develops in partnership with internal and external stakeholders an evidence-based approach to meet the complex health needs of county, City of Fairfax, and Falls Church residents;
  • Creates a comprehensive care coordination program with clear metrics and objectives in concert with internal and external county partners to facilitate a seamless continuum of care;
  • Facilitates the development and monitoring of system performance/outcome measures and works with safety net partners to evaluate the implementation of system-wide objectives;
  • Develops and expands an integrated dental care safety net program as part of the safety net;
  • Builds, in collaboration with external partners, a comprehensive specialty care network and referral process;
  • Manages and monitors the performance of key county contractors that provide primary care, specialty care, dental care, and pharmacy support services for county residents.
  • Supports the strategic efforts and priorities of OSM to realize our mission.


Illustrative Duties

Plans, organizes, directs and manages the county's health safety net and integration efforts;
Develops and implements standards, policies and procedures for the delivery of the an integrated health service delivery;
Oversees business (service) processes ensuring full integration of primary care, behavioral health, dental care, pharmacy services, and specialty care;
Develops in partnership with internal and external stakeholders an evidence-based care coordination program to improve the continuum of care offered to uninsured and underserved populations;
Oversees the monitoring of service utilization, financial performance of the safety net system;
Responsible for the monitoring of the safety net's administrative processes to include eligibility determination, contracts, negotiations and data collection;
Collaborates with county and community providers to integrate models to enrich patient outcomes and experience;
Establishes and maintains strong provider relationships to ensure lines of communications are open and strong;
Develops and maintains ongoing relationships with key internal partners;
Monitors and meets organizational and budgetary goals and performance;
Oversees, develops and monitors requests for proposal (RFP's) as necessary;
Serves as a liaison to relevant Boards, Authorities, and Commission on the county's safety net and health integration work;
Conducts surveys and studies for special projects relating to the safety net system;
Presents to professional, civic and community groups as appropriate.

Required Knowledge Skills and Abilities

Knowledge of health care, safety net population, and delivery models
Knowledge of public health practice;
Knowledge of the principles and practices of public administration (including personnel, budgeting, and management analysis), and the ability to apply them;
Knowledge of Federal, state and county laws, and regulations affecting safety net programs;
Knowledge of social determinants that influence the health of the community and strategies to achieve health equity;
Ability to advance health equity, address systemic health disparities and collaborating with diverse, vulnerable and underrepresented populations.
Ability to develop and apply new insights, innovative solutions and evidence-based approaches to ensure an effective and accountable safety net;
Ability to analyze data and draw sound conclusions;;
Ability to effectively coordinate and implement goals and objectives;
Ability to establish strategic community partnerships through community engagement;
Ability to identify interrelationships across department divisions and to leverage internal and external resources;
Ability to plan, direct and coordinate the work of staff both directly and indirectly;
Ability to provide effective leadership;
Ability to communicate clearly and concisely, orally and in writing;
Ability to develop and maintain effective working relationships with subordinates, coworkers, County officials, public and private sector organizations, community groups and the public.

Employment Standards

EMPLOYMENT STANDARDS:
Any combination of education, experience, and training equivalent to the following:

Graduation from an accredited college or university with a Master's Degree in health care administration, health systems management, public health, social work or related field, plus five years of progressively responsible experience in professional-level contract management, evaluation, analysis and coalition-building that includes the development and oversight of safety net practices and programs.

NECESSARY SPECIAL REQUIREMENTS:
The appointee to this position will be required to complete a criminal background check, and sanction screening to the satisfaction of the employer.

PREFERRED QUALIFICATIONS:
Master's degree preferred in a related field with experience in health care integration and safety net design and monitoring; systems thinker with an ability to bring both internal and external partners together; superior interpersonal and communications skills.

PHYSICAL REQUIREMENTS:
Job is generally sedentary in nature; however, job entails walking, standing, sitting, climbing stairs, reaching and bending; uses hands to grasp, handle, or feel. Visual acuity is required to read data on a computer monitor; ability to drive a vehicle. Employee may be required to lift up or carry up to 25 lbs. occasionally. All duties perform with or without reasonable accommodations.
