The Fairfax County Health Department (FCHD) is a progressive public health leader providing services that promote population health, protect public health and the environment, and ensure residents' access to health services and information. Under general supervision of the Public Health Nurse IV, the Center Nurse Coordinator (CNC) is responsible for the daily management and oversight for a specialized population of individuals, 18 and older, with cognitive and physical diagnoses, who are served in an Adult Day Health Care Center (ADHC). Responsible for ensuring operational compliance with state and local regulations. The CNC uses a person-centered approach to the provision of direct services and care coordination through collaboration with the primary care provider, family caregiver and other service providers to meet the pyscho/social/physical needs of the ADHC participants. The CNC manages the center's fiscal/human/material resources, to ensure provision of quality services within allocated funds. Provides coaching and performance management of staff; evaluates the efficiency and effectiveness of the center based on established performance metrics. Employment Standards

MINIMUM QUALIFICATIONS:
Graduation from a college or university accredited by the National League for Nursing or the Commission on Collegiate Nursing Education with a bachelor's degree in nursing; plus three years of post-licensure experience as a nurse in an equivalent health care setting whose focus was primarily prevention services and community education, including two years in a public health/community health setting; plus at least one year experience in a public health/community health specialty area or supervisory responsibilities.

CERTIFICATES AND LICENSES REQUIRED:
Possession of current license or a multistate licensure privilege to practice as a Registered Nurse in the Commonwealth of Virginia
Driver's License
AED-Required within 60 days
CPR -Required within 60 days

PREFERRED QUALIFICATIONS:
Preferred candidate will have a minimum of five years' experience providing health services to geriatric patients, be well-trained and knowledgeable in the area of dementia care. Must possess at least three years of direct supervisory experience or have demonstrated work experience monitoring and directing the work of others. Ability to communicate effectively and deal tactfully with clients of diverse ethnic, cultural, and economic backgrounds. Models outstanding customer service skills with the ability to establish and maintain effective and cooperative relationships. Proficiency in the use of Microsoft Office Suite and other information systems such as an electronic health records system. Bilingual Spanish/English a plus.

NECESSARY SPECIAL REQUIREMENTS:

The appointee to this position will be required to complete a criminal background investigation, a Child Protective Services (CPS) check, a driving record check, and a sanction screening to the satisfaction of the employer.

PHYSICAL REQUIREMENTS:
Employee is required to walk, stand for long periods of time (i.e.: 1 to 2 hours), bend, sit, kneel, reach, climb stairs and participate in recreational therapy. Uses hands to grasp, handle or feel. Visual acuity is required to read date on a computer monitor. Ability to operate keyboard driven equipment. Ability to carry equipment and assist with client transfers from sitting to standing and lift up to 30 pounds. Ability to drive a vehicle. All duties performed with or without reasonable accommodations.
