The Manager, Care Navigation is responsible to operationalize the strategic plan for the department, ensuring collaboration with patients and their care team so that patients are navigated through the continuum of care, within the Adventist HealthCare system when possible. The manager will role model professionalism and exemplary standards of practice while mentoring, evaluation, and facilitating the professional growth of their team. Performs principle duties and responsibilities in a manner consistent with the Organization's mission, values and goals.

Responsibilities:
• Supports departmental and organizational financial goals of minimizing potentially avoidable utilization and reducing readmission rate
• Works collaboratively with the acute care teams, post-acute care teams, and physician practices to ensure patients care is coordinated to reduce avoidable utilization
• Oversees delivery of care provided by Care Navigation team to ensure patients' needs are addressed and documentation meets standards
• Role-models positive customer service behaviors through the consistent application and deployment of AHC's Values and Mission and holds self and others accountable
• Collaborates in the development of the strategic plan and its execution
• Coordinates staff competency-based orientation and continuing education activities. Evaluates effectiveness of orientation and training, implements appropriate changes based on evaluation.
• Oversees and evaluates the performance of the Care Navigation team
• Facilitates the development of systems, processes and PI that drives optimal performance for the department

Qualifications:
• Bachelor of Science in nursing required, Master's degree preferred
• 3 - 5 years' experience in patient navigation, discharge planning, or care coordination
• At least one year of prior leadership experience required
• Maryland RN licensure
• Able to mentor and transform the performance of groups in a manner consistent with the organization's mission and values
• Effective communication and interpersonal skills
• Demonstrates excellent collaboration and with both internal and external customers and stakeholders; able to maintain and manage customer relationships
• Application of evidence-based practice and performance improvement techniques
• Maintains a climate that attracts, retains and motivates staff

Work Schedule:

Monday through Friday with occasional evenings or weekends as required. Must have a reliable vehicle.
