JOB DESCRIPTION

Purpose of Position:

The ED Community Health Worker reports to the Manager of Care Transitions and Management within Adventist HealthCare.

The ED Community Health Worker works in the Emergency Department (ED) to identify and provide resources necessary to allow patients to manage their health outside of the ED. The ED Community Health Worker supports ED patients and staff to coordinate medical and social resources to promote safe and efficient discharge plans.

Principle Duties and Responsibilities:

1. Identify patients who are vulnerable, high risk for readmission within 30 days of discharge, and/or high ED utilizers requiring additional support and connections to community resources.
2. Collaborate with patients, ED staff, ED physicians, and community providers to develop a comprehensive plan of care for safe discharge. Completes required documentation in a timely and accurate manner.
3. Collaborate with behavioral health team and other community groups to ensure patients are connected to the most appropriate mental health programs in the community.
4. Make follow up calls for recently discharged patients to ensure they are following through with the discharge plan.

Strong organizational skills; able to develop workflows to create more efficient processes
Ability to work in a fast-paced environment while managing multiple priorities and maintaining professionalism
Excellent time management skills
Ability to problem solve and exercise good judgment

Education:

Bachelor of Science in health education, health promotion, or related field

Experience:

1- 2 years' experience working in a community-based setting with knowledge of local resources and system navigation; experience in the ED is preferred

Work Schedule:

Work every weekend
