Responsibilities

The HBPC NP is responsible for routine and advanced care and will perform examinations of Veterans and residents for determination of changing conditions or verification of medical or mental status. HBPC is highly visible and requires a dedicated and self-motivated individual to maintain a favorable public image. The NP has the responsibility for coordinating activities of the VA staff performing HBPC/Pact team duties as assigned. To include timely and cost-effective support services (administrative, finance, budget, etc.) necessary to meet all accountabilities including new program implementation and development of HBPC program areas.

Examples include development of performance standards, writing position descriptions, and evaluating work performance. The NP will also provide cross coverage for other HBPC Providers in/on other sectors/pact teams in the event of staff shortage, leave, or vacancies within the department.

1. Examinations include medical record, diagnostic review and also include physical examination of the patient, ranging from simple to complete full body physical assessment.
2. Assists with completion of peer reviews and assessments such as OPPE/FPPE chart reviews.
3. Assists in the development and maintenance of reports, graphs and other forms of analytical tools to inform and aid in decision making at the executive level.
4. Demonstrates advanced competencies and skills in assessing the physical and psychosocial status of Veterans as needed to meet performance measures and standards.
5. Obtains a timely complete health history, physical, and appropriate screening measures.
6. Orders appropriate diagnostic tests in accordance with privileges
7. Initiate and modify therapies based on initial and ongoing assessments and accepted standards of practice.
8. Provides initial and episodic assessments to monitor acute and chronic illnesses.
9. Attend and present cases appropriately at Interdisciplinary team meetings.
10. Initiates and modifies consults/ referrals based on initial and ongoing assessments to appropriate specialty clinics and community services.
11. Reviews consults/referrals to determine urgency and complexity of Veteran health care needs
12. Provides appropriate preventative health education, intervention, and documents timely and appropriately
13. Demonstrates the ability to manage/schedule/cluster Veteran visits to ensure workload and productivity goals are met.
14. Complete timely, clear, and concise documentation in the Veterans health records or other program related correspondence
15. Advise on, administer, supervise, or perform professional and developmental work.
16. Will assist in development of Executive and Congressional requests for information on all issues related to HBPC.
17. Assist the HBPC /GEC Leadership in projects as assigned.
18. All other duties as assigned

Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 10/25/2019.

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing ( ACEN ) or The Commission on Collegiate Nursing Education ( CCNE ). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Preferred Experience:
  • Graduate of an accredited school of nursing with Masters Degree in Nursing (MSN).
  • At least 3-5 years (within the last 3 years) nursing experience in Primary Care, Geriatric/LTC or Managed Care. Home Health care experience preferred. Must be able to practice in a minimally supervised practice area and exhibit strong interpersonal, care coordination/case management and leadership ability.
  • Additional requirements include phlebotomy, urinary catheterization, wound care, g-tube and ostomy care experience is required for this position.
  • Strong patient/family teaching experience required.
  • Excellent documentation and communication skills.
  • BLS (Basic Life Support).
  • Strong physical assessment and critical thinking skills and the ability to work autonomously are also required for this position.
  • Required - A valid Maryland driver's license (or the state in which the RN physically resides); the physical ability to independently drive a car, walk up long, narrow flights of stairs, assist patients as needed as part of the assessment process, carry heavy medical bags and equipment up those stairs as needed for patient care, lift 50 or greater pounds as required for patient care, be able to tolerate a variety of weather conditions and environmental conditions in a Veteran's place of residence.


Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
  • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
  • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.

Physical Requirements: It is VA policy to ensure that all selected applicants and employed personnel are physically, mentally and emotionally fit to perform the duties of the position to which assigned. A pre-placement physical examination is required for all fulltime, part-time and intermittent employees identified by VA Directive and Handbook 5019. The pre-placement physical examination shall be completed prior to appointment to determine the physical and mental fitness for candidates for appointment in VA. The examination will be concerned with the mental and physical ability of the applicant to satisfactorily perform the duties of the proposed assignment and will include requisite laboratory and other screening as required by Federal regulatory agencies.

This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. Annual leave shall accrue for full-time Nurses, and Nurse Anesthetists at the rate of 8 hours for each full biweekly pay period.
