Manages Admissions and Social Services Functions, provides Department leadership and supervises staff. Directs and manages all resident discharge functions and the social services activities provided to facility residents. Acts a liaison between residents, families and staff of the facility. Assumes responsibility for the discharge and psychosocial needs and interests of residents.

Required: Social Work in Long Term Care - 1 year

Elderly Community Resources experience required. Medical Charting experience required. Medicare experience required. Medicaid experience preferred. Third Party Payers experience required. For Nursing and Assisted Living facilities a BLS Required. PACE employees require a CPR card.
