This role will kick off, coordinate, establish, validate and maintain data integrations within our community providers.

1. Run and coordinate all aspects of community practice connections to our population health
2. Connect with practices and EHR vendors and explain data requirements and answer
3. Liaise with community practices to be able to validate data connections
4. Coordinate all internal resources needed for data integrations
5. Maintain data points on connection status and report out to stakeholders.
6. Perform manual data extractions for groups where applicable
7. Establish and maintain documentation on all connections and procedures
8. Understand the business application and database supporting the applications
9. Establish regular projects calls with vendors on integrations and attend weekly project calls
with internal groups to explain progress

1. Must be extremely organized
2. Ability to handle multiple connection projects and prioritize accordingly
3. Ability to develop successful relationships with external and internal partners
4. Excellent oral and written communication skills
5. Demonstrated problem solving, multi-tasking, troubleshooting skills with a high degree of flexibility

Education: Bachelors Degree required; Computer Science,Math, Engineering, MIS degrees are preferred

Minimum 3 years experience in healthcare, data sets or IT experience Excellent analytical and research skills.
Strong technical aptitude with the ability to analyze data, integrate data.
Proven pro-active, self-starter with the ability to work both as an individual and within a team to resolve issues.
Ability to work on multiple projects and deliverable simultaneously. Proficiency in Microsoft Office Professional.

Other: Licensed driver and able to operate a motor vehicle as well as maintain automobile insurance

Work Schedule:

M- F 8 am - 4:30 pm Must have reliable transportation
