This position will design, build, staff, and supervise Advanced Practice Providers in delivering clinic and home-based medical services to frail Medicare, Medicare Advantage, and Transitions patients of Sentara Medical Group. This position will grow and lead a team to reduce unnecessary hospitalizations and overall cost of care, and to improve the quality of life of the senior adult patient population. Lead team in improving outpatient management of chronic diseases, delivering evidence based preventive care, providing patient and family support and education, and optimizing utilization of health care services. Responsibility for leading activities to improve Medicare Risk Adjustment, including oversight of professional coders and Provider coding coaches. Serves as a key leader in developing and building the Complex Care Platform across all medical groups, developing strategies and best practices to improve outcomes for complex care patients.

Education Level
Master's Level Degree - NURSE PRACTITIONE

Experience
Required: Frail and Elderly Population - 2 years, Management - 2 years, Nurse Practitioner - 3 years

Preferred: Population Management - Previous experience

License
Required: Nurse Practitioner
