Job Summary : Responsible for supervision of the Bereavement Program. Provides planning and oversight and implementation for all functions of the grief support programs and services for 13 months following death of hospice patient (per CoPs). Duties and Responsibilities: Manages and directs all grief support programs and bereavement activities to ensure timely an appropriate services are provided. Plans for Bereavement Program development, expansion and refinement through annual evaluation of services, implementation of new bereavement support programs. Provides bereavement support to hospice personnel coping with work related grief through one-to-one counseling, supervision of interns providing one-to-one counseling, and referral to community resources. Coordinates/completes assessments of clients, families for emotional, spiritual, bereavement needs and develops an individualized plan of care. Maintains bereavement client files. Documents all patient/family contacts and/or bereavement sessions per policies and regulations. Insures family needs are met by referrals to appropriate individual counseling or support groups. Maintains oversight of and reviews bereavement records, logs, mailings to maintain compliance. Develops, provides and directs bereavement services to the community in the form of counseling, presentations, support groups, children's camp or other activities/programs as needed. Creates annual grief support activities calendar. Attends and participates in IDT meetings to assist in developing and updating Plan of Care. Assess for anticipatory grief needs for families, children, caregivers and develops a bereavement plan of care. Works with the school system, crisis programs and Health Department and other agencies to evaluate and provide bereavement services to children. Develops and tracks statistical information regarding utilization of services and program outcomes. Facilitates recruitment, supervision, training of volunteers or other bereavement staff to ensure compliance with the bereavement program. Demonstrates community engagement through events and educational presentations, bereavement support, referral services to increase awareness and access to grief support services for the entire community. Provides phone support/services for crisis response in the community. Participates in hospice activities such as inservice education, hospice personnel meetings, and relevant committees. Actively participates in quality assessment performance improvement teams and activities. Responsible for annual review an update of bereavement policies. YourMembership.
