Description: The Clinical Social Worker (Part-Time) is a member of the Bethesda Cares team responsible for implementing housing-focused outreach methods to assist populations experiencing homelessness. As a housing-focused outreach program, all engagement is intended to build trusting relationships and confidence with clients to raise their level of care beyond homelessness. Requirements - State of Maryland LCSW-C (Licensed Certified Social Worker - Clinical)Clinical Social Worker's duties and responsibilities include: Identifying and engaging persons experiencing homelessness as defined by HUD criteria - persons living in a place not meant for human habitation -- particularly those who may not independently seek services;Assessing the individual's safety, immediate needs, level of functioning, overall mental and physical health, strengths and primary support system;Determining if individuals experiencing street homelessness meet criteria for clinical diagnosis and documenting all information to support diagnosis determination;Utilizing Housing First principles to make recommendations for raising the level of care for each client engaged by the street outreach team while evaluating and prioritizing persons identified as chronic homeless for evidence-based permanent supportive housing referral and placement;Maintaining and interacting in a culturally sensitive, respectful and professional demeanor;Working intensively in one-on-one relationships with clients to provide support, engagement, and referral;Utilizing Bethesda Cares platforms for client engagement such as street outreach, street assessments, meal program, and drop-in lobby;Maintaining records in Homeless Management Information System (HMIS), including demographics, case summary notes, transactions and assessments in timely manner;Coordinating and collaborating with other homeless services providers and community resources to facilitate access to the appropriate resource, including basic needs (food, clothing, shelter, hygiene, and laundry), income entitlements, housing assistance, mental and somatic health treatment, substance abuse education and treatment, legal assistance and health information;Maintaining outreach/working relationship with all stakeholders, urban district staff, police, health providers, churches, and other groups who might provide insight and information related to persons experiencing homelessness;Being thoroughly familiar with the policies and procedures guiding the work of Bethesda Cares housing-focused outreach and performing job functions in line with guidelines;Attending regular training sessions to enhance best-practice knowledge in the field of homelessness, Housing First, and permanent supportive housing resources and providing staff education on best-practice and evidence-based services;All other duties as assigned.
