Summary

The Licensed Practical Nurse works under the direction of an RN and reports to the Nurse Manager, Primary Care. The LPN is responsible for observing, recording, and reporting changes in the patient's conditions, as well as assisting in the development and revision of the patient-driven holistic car plan for life.

Responsibilities

Duties include, but are not limited to, Makes observations and thoroughly documents care and the patient;s response; seeks assistance and guidance by informing RN or medical provided of change in the patient's condition requiring higher-level intervention. The LPN preforms, reports, and records assigned nursing care in the Primary Care Clinics, including but not limited to:Oxygen therapy, medication administration to include IV medication, if certified; clinical reminders with appropriate consults and alerts as indicated; check expiration dates of meds and supplies; delegates procedures and treatments and documents.Assists with minor procedures, including obtaining and arranging necessary equipment , positioning patient, ensuring comfort and privacy and monitors patient during the procedure. Adheres to standards of practice and safe techniques in preparation and administration of medication. Adheres to standards of practice and sage technique in preparation and administration of medication, recognizes urgent or emergent patient care situation, and seeds assistance and effectively intervenes as directed when appropriate.Adheres to principles of infection control, safety, and universal precautions, possesses knowledge of proper handling and pre-cleaning of reusable medical equipment

Qualifications

BASIC REQUIREMENTS: United States Citizenship; English proficiency, and current BLS certification.
LICENSURE: Current, active and unrestricted LPN license from any U.S. state or territory.
EDUCATION: Graduation from a school of practical nursing approved by the appropriate State accrediting agency or the Accreditation Commission for Education in Nursing (ACEN) at the time the program was completed.English

The full performance level of this vacancy is 6. The actual grade at which an applicant may be selected for this vacancy is in the range of 3 to 6.

GRADE DETERMINATIONS:
GS-03: None beyond the basic requirements.

GS-04: Six (6) months of qualifying experience as an LPN or LVN; OR Graduation from an approved school, and one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures; OR Graduation from an approved school of at least 24 months duration.

GS-05: Completion of at least one (1) year of qualifying experience at the GS-04 level or equivalent. To qualify at the GS-05 level, applicants must demonstrate the following competencies:Demonstrated knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems.· Demonstrated ability to serve as a responsible member of the nursing team and interact with patients, family members, professional and supportive personnel involved in the delivery of patient care, incorporating acceptable, established customer service standards into practice.

GS-06 (Full Performance Level): Completion of at least one (1) year of additional qualifying experience at the GS-05 level or equivalent. To qualify at the GS-06 level, applicants must demonstrate the following competencies:Technically proficient in initiating, performing and completing assigned duties in providing care to variable patient populations.Knowledge and ability to appropriately carry out assigned patient care based on the patients' conditions; to use judgment in selecting the appropriate order and sequence of procedures and treatments; and to accurately recognize, report and record relevant patient information.Prepare and administer prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and perform treatments according to established policies/procedures. Observe for physical and/or emotional changes in patient's condition from prescribed medications/treatments and reporting any deviations from normal to RN or MD/DO. Physical Requirements: Must possess mental and emotional stability. Functional requirements include the following abilities: Heavy lifting of 45 pounds and over, heavy carrying 45 pound and over, Pushing 4 hours, Reaching above shoulders, use of fingers/ both hands are required, walking for 8 hours, standing for 8 hours, both legs are required and kneeing and bending, ability for rapid mental and muscular coordination simultaneously, near vision correctable in one eye to 20/20 and to 20/40 in the other eye, ability to distinguish shades of color, hear the whispered voice (aid permitted). Environmental Factors required: Working with hands in water, working closely with others and protracted or irregular hours of work.
