Social Worker Case Manager The Clinical Social Worker is responsible for the assessment, planning, implementation, coordination, monitoring and evaluation of the patient's plan of care from admission to post-discharge. The Social Worker utilizes clinical knowledge, critical thinking skills, and the principles of case management to coordinate and implement a discharge plan that meets the patient's needs and ensures a seamless, effective and efficient transition of care across the continuum. : Job Requirements : Education/Qualification Current Clinical Licensure in Social Worker (LICSW) in the District of Columbia. Attendance and Punctuality - Demonstrates the ability to arrive at work site on-time; limits the number of absences; provides adequate notice to supervisor for time off; takes lunch periods and breaks within prescribed times Mandatory training, testing, and competencies: completed on time - Competency Training; Services Excellence Training; Annual Physical Master's degree in Social Work Three years experience in acute care or other related healthcare field is preferred Case Management Certification preferred. Previous experience in case management and/or discharge planning is strongly preferred. Skills Effective oral and written communication and interpersonal skills Knowledge of government and third party reimbursement systems and contracts.
