Job Announcement

Provides Medical Respite Services for the department. Assumes a cooperative role in providing medical casework services, which involves working as a partner with the Health Department, the Office to End and Prevent Homelessness, and the Community Homeless shelters. Receives referrals for the program and conducts eligibility assessments. Emphasis of the assessment is to understand client's health status and primary health care needs. Casework services include conducting a psychosocial assessment, and in collaboration with the client and the multidisciplinary team, developing a client centered, solution focused care plan to meet psychosocial, health, emotional, financial, and housing needs. Assists with arranging transportation services and the filling of prescriptions; purchases over-the-counter medications. Arranges for or assists with emergency services. Assists clients with receiving public assistance benefits, disability income, health insurance, and legal services. Provides therapeutic counseling and crisis intervention. Conducts outreach, community education and consultation regarding the program. Maintains records using required systems. Attends required meetings and participates in training.

Employment Standards

MINIMUM QUALIFICATIONS:
Graduation from an accredited four-year college or university with a bachelor's degree, plus two years of professional social work experience. A master's degree in a related field may be substituted for one year of the required experience.

Contingent upon the area of assignment, some positions within this class may require specific certification(s) prior to employment and/or the ability to acquire necessary certification(s) within a specified time period following employment. The certifications are identified in the position description and employment advertisement .

CERTIFICATES AND LICENSES REQUIRED:
Driver's License (Required upon hire).

NECESSARY SPECIAL REQUIREMENTS:
Appointee must satisfactorily complete a criminal background check, a check of the Child Protective Services Registry, and a driver's record check to the satisfaction of the employer.

PREFERRED QUALIFICATIONS:
  • Master's degree in social work (MSW).
  • Education or experience in law, nursing, psychology, or other related field.
  • Professional advocacy and/or ombudsman experience.
  • Experience in the field of aging, long-term care, homeless services, and/or volunteer management in related fields.
  • Demonstrated understanding of the needs of frail adults, services targeted for them, and regulations that apply.
  • Experience and/or interest in governmental policy and legislative advocacy work; training, public education and negotiation skills.
  • Proficient in the use of computers and databases; experience with database management software and Microsoft Office Suite applications (i.e., MS Word, Excel, and Access).
  • Clear and concise oral and written communication skills and public speaking skills.
  • Ability to work in fast-paced and demanding environment.
  • Experience working with diverse multicultural populations.
  • Detail-oriented and organized person with high-level skills in public speaking, networking, coordinating, data entry and multi-tasking.
  • Multilingual in Spanish.

PHYSICAL REQUIREMENTS:
Ability to use automated technology. Ability to lift 10 pounds. Ability to communicate effectively both orally and in writing. All duties performed with or without reasonable accommodations.
