Job Responsibilities

In collaboration with the Transitional Care RN, coordinate patient care with patient, family, and ancillary/community services to promote quality and cost-efficient care for assigned caseload.

Completes required documentation in a timely and accurate manner.

Assist patients with enrollment and/or referring them to various health and social service programs. May include meeting patient in community to assist with connection when needed.

Assist patients to schedule medical appointments to include connection with transportation resources.

Provide follow-up to patients, by phone, home visits and visits to other settings where patients can be found.

Conduct home visit to ensure that the patient has what is required to recover safely at home.

Visit to include safety checks (e.g., check for fall risks) and preparation for follow-up appointments with provider.

Behavioral health experience preferred
Empathetic toward individual's complex health needs and their families
Strong communication and customer service skills
Knowledge of community resources
Strong writing skills
Strong organizational skills; able to develop workflows to create more efficient processes
Ability to work in a fast-paced environment while managing multiple priorities and maintaining professionalism
Excellent time management skills
Ability to problem solve and exercise good judgment

Bachelor of Science in health education, health promotion, or related field
1 - 2 years' experience working in a community-based setting with knowledge of local resources and system navigation
Licensed driver and able to operate a motor vehicle as well as maintain automobile insurance

Work Schedule:

Position may require holiday, weekend, evening rotation when needed to support patient care
