RN Transition Coordinator (Manassas,VA)(Gainesville,VA)(Herndon,VA)(Sterling,VA) Commonwealth Care of Roanoke, Inc. (CCR) is currently searching for a dynamic RN Transition Coordinator. Reporting to the Regional Director of Operations, this position will be responsible for admission decisions for hospital and/or health care center referrals and the coordination of the transfer and admission of patients to the Center. Additionally, the RN Transition Coordinator will ensure business growth through creating and maintaining effective partnerships and relationships with key healthcare entities and personnel that is supportive of our company organization strategy, mission, vision, and core values. We are looking for an individual to lead the transition care process to meet community needs and our company goals. This position supports multiple centers in the Northern VA area including Sterling, Herndon, Gainesville and Manassas . CCR, Inc. manages twelve health and rehabilitation centers across the Commonwealth of Virginia. Our centers provide a resident-centered, customer service approach to care by qualified staff that values quality and commits to compliance and higher standards. Successful service excellence candidates will have the following qualifications and ability to: Facilitate relationship building and maintaining partnerships with hospitals and referral sources. Conduct proactive rounding and on-site visits at the hospitals and health care centers. Act as an initial contact for admission decisions. Conduct pre-placements clinical and financial screens making quick decisions on appropriate placement. Collaborate with patient or representative, physicians, hospital staff, discharge planners, and center staff to ensure a successful, seamless transfer to the facility. Convey accurate patient care information inclusive of physician orders for admission. Identify patient care, equipment, personal and educational needs and convey to center staff. Regular communication with facility regarding appropriate placement, admission, room assignment, and roommates and census updates. Develop and maintain admission and marketing strategies for the centers. Assist in identifying training needs for center TCU staff to eliminate clinical barriers to admission. Participate in QAPI or other process meetings with the teams upon request. Collaborate with the VP of Clinical Services and corporate staff to ensure safe transitions or complex patients. Must be an advocate for patient interests. Promotes a positive and collaborative service excellence team environment. Demonstrate and promote a culture of Service Excellence. Perform other duties as assigned. EOE and Drug Free Requirements Current RN license in good standing in VA. Previous health care experience in an acute hospital or long-term care setting. Strong interpersonal skills. Demonstrated experience in working collaboratively with physicians, discharge planners/case manager, and staff. Strong leadership, clinical and customer service skills. Prior leadership experience preferred. Must be flexible, detail-oriented, have the ability to multi-task and be able to make effective decisions and work as part of a service excellence team. Proficient in Microsoft Office (Excel, Work, Outlook). Use your nursing and organizational skills to successfully work with a dedicated team!
