Job Summary: The Level II Competent Registered Nurse is clinically able to care for patients. The Level II MWHC nurse demonstrates the ability to collaborate with the healthcare team to improve the quality of care in the designated clinical area by incorporating research and implementing evidenced-based knowledge into practice. Essential Functions & Responsibilities: In the MWHC Practice Standards (attached) the essential functions and responsibilities of a Level II Registered Nurse is outlined in the following areas of: Exemplary Professional Practice Standards Complete assessment/intake screening with recognition of abnormal, pertinent findings. Document and communicate assessment results across shifts/episodes of care with recognition of potential safety issues to the team. Develop plan of care with the interdisciplinary team based on medical, nursing and patient/family input. Document in accordance with practice standards. Evaluate patient changes in condition and modifies plan of care appropriately. Structural Empowerment Practice Standards Advocate for patients and families with ethic consults as needed. Transformational Leadership Practice Standards Lead the team related to patients assigned: CNA/techs, credentialed provider team members, rounding, initiating patient/family conferences. Participate on unit council/committees. Promote positive team relations and communication. New Knowledge, Innovations, and Improvement Practice Standards Detect nurse sensitive NDNQI (National Database of Nursing Quality Indicators) outcomes for unit/clinic: falls, skin, etc. Identify areas for unit improvement and how care could improve utilizing FOCUS-PDCA (Find, Organize, Clarify, Understand, Select/ Plan, Do Check Act). Assists with completing regulatory testing within time requirements. Qualifications: Graduate of an accredited nursing program and has experience as a Registered Nurse. NOTE: PRN position must have a minimum of two (2) years of experience as a Registered Nurse Registered Nurse licensed or eligible to practice in Virginia. BSN is preferred; all new Registered Nurses with an Associate's Degree in Nursing will be required to obtain their BSN within five years. CPR, required Home Health April 2017 a. Possess a current, valid driver's license; must meet the driver suitability criteria for MWHC. b. 1 year home health or medical/surgical nursing experienced required. c. Provides for the physical, mental and social well-being for individuals and families by assessing, planning, implementing and evaluation for nursing care. d. Performs the comprehensive assessment and develops/implements and evaluates the plan of care as identified on admission and subsequent home visits. e. Provides appropriate and through education to patients and their caregivers as it relates to the plan of care. f. When assigned, serves as case manager to coordinate multidisciplinary services per the plan of care. g. Maintains acceptable productivity standard as determined by agency leadership. h. Participates in multi-disciplinary team meetings in order to share information and assist with patient management. i. Participates in staff and administrative meetings. j. Participates in quality and performance improvement activities.
