Job Summary:

The Director of Performance Improvement develops, maintains, and evaluates all aspect of the organization to enhance performance /quality improvement, in collaboration with the company's management. The Director of Performance Improvement develops and implement strategies to ensure the delivery of quality health care and services. Works collaboratively with the management team inclusive of the clinical reviewer to assist the organization in achieving the desired outcomes, minimizing and /or mitigating risk.

  • High competency level in regulations, standards guidelines associated with DHMH Residential Service, Maryland Medicaid, District of Columbia Home Care licensure guidelines, CMS Home Care regulations and CHAP Standards of Excellence for Home Care.
  • Participates and coordinates quality improvement performance measures including but not limited to:
    • Analysis (Quantitative and Qualitative)
    • Identification of opportunities for improvement
    • Devising, implementing, evaluating strategies (interventions) to improve quality of care/service.
    • Barrier analysis
  • Maintains expertise of national certification/accreditation standards, Federal and District of Columbia statutes, Maryland State regulations related to DC/MD Board of Nursing, Maryland Medicaid Provider Enrollment, Residential Service Agencies, CMS guidelines related to home care, as well as contractual obligations.
  • Works collaboratively with the Nurse Educator/Reviewer, Office Manager to guide regulatory required education, performance/quality improvement opportunities /initiatives.
  • Participates in a supportive role in CMS, DOH, CHAP, DHMH accreditation surveys.
  • Performs 10% or minimum of 10 random medical record reviews monthly
  • Trends data to support targeted employee/ client /caregiver and provider educational interventions to support and improve specific QI/QP initiatives.
  • Serves as the lead in collaboration with the Clinical Director, Nurse Review/Educator and Health Administrator a in preparing mortality record review and associated corrective action plans, inclusive of medical record request.
  • Works in collaboration with the Health Administration Manager, Clinical Director and Nurse Reviewer to ensure accuracy of authorizations and scheduling.
  • Works with internal staff and external providers to communicate CHAP standards, DOH Review, CMS reviews and elicit the necessary cooperation for its successful implementation.

  • Two (2) to three (3) years of experience in developing, implementing, and evaluating clinical QI/PI initiatives Three (3) to five (5) years of professional clinical experience.
  • Must have working knowledge of Community based practice, Home Care.
  • Ability to work under pressure, strong organizational and analytical skills is required.
  • Demonstrated project management skills with the ability to handle multiple concurrent projects.
  • Ability to work effectively with providers and multidisciplinary teams across departmental lines.
  • Familiar regulatory compliance, CMS/DOH/DHMH. Excellent written and verbal communication skills.
  • Must have excellent computer skills, Knowledge applications MS Suite (Word, Excel, PowerPoint, and Outlook).


CPHQ must be obtained within 12 months of employment; Current DC Licensure and Maryland Licensure.

Required Education:

Master's Degree required MS, MSN, MPA, or MPH.
