Summary

About the Position: This position is located at Walter Reed National Military Medical Center, Bethesda, MD. Work is performed in a hospital based ambulatory clinic, where exposure to contagious and infectious disease is common. Tour of Duty is generally an 8 hour shift between the hours of 0730 and 1800 Monday through Friday.

Responsibilities

  • The applicant will be expected to lead the Patient Centered Medical Home team overseeing the clinical care of patients, as well as meeting goal based metrics.
  • Oversee the population health needs of an enrolled panel of 3000-5000 patients.
  • Provide nursing care for all ages, within the Medical Home Port Team (MHPT) and Patient Centered Medical Home (PCMH) environment, utilizing evidenced-based clinical practice guidelines.
  • Encourage patients and their family to actively participate in patient health care needs.
  • Directs, plans, coordinates, implements, and evaluates activities, relevant to the delivery of ambulatory nursing care, within the Medical Home Port (MHP) clinic.
  • Provide a variety of consultative services, to include practitioner referral and evaluation, health care advice, and initiation of medical referral, based on medical staff protocols.
  • Initiate continuous process improvements to improve access to quality of care delivered experience of care, and the utilization of health services by enrollees.
  • Utilize automated programs and information technology tools to facilitate the patient experience.
  • Identifies clinical, administrative, and staffing problems and independently utilizes problem-solving techniques to ensure the delivery of quality patient care.
  • Plan and implement performance improvement activities, based on the process improvement plan for the medical treatment facility.

  • This position requires the incumbent be able to obtain and maintain a determination of eligibility for a Tier 3 security investigation or access for the duration of employment. A background investigation and credit check are required.
  • This position requires the incumbent to have tuberculosis testing.
  • Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation.
  • Male applicants born after December 31, 1959 must complete a Pre-Employment Certification Statement for Selective Service Registration.
  • Two year trial/probationary period may be required.
  • This position has mandatory seasonal influenza vaccination requirements and is subject to annual seasonal influenza vaccinations unless otherwise exempted for medical or religious reasons (documentation of exemption must be provided upon hire).
  • This position is subject to a pre-employment drug screening, and random testing thereafter, to include testing based on reasonable suspicion and testing due to direct involvement with an on-duty accident.
  • This position requires the incumbent to fulfill credentialing requirements and obtain and maintain appropriate/relevant clinical privileges.
  • This position requires that you obtain and maintain a Basic Life Support (BLS) certification.

Basic Requirement for Nurse (Consultant/Medical Home Port Team/Patient Centered Medical Home):

Education: A graduate or higher level degree, bachelor's degree, associate's degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.

Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.

Licensure: Applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.

In addition to the above requirements, applicants must meet one of the following requirements:

Experience: At least one full year of professional nursing experience (equivalent to the GS-9 grade level) which includes conducting patient assessments in an out-patient setting; providing professional ambulatory nursing care; and educating patients and family members on treatment plans.

OR

Education Substitution: Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field.
