Purpose of Position - Essential Functions (the fundamental, crucial job duties performed in this position):

The Intake, Care Transition Coordinator (CTC) interfaces with existing and potential patients, family members, referral sources and physicians to educate and facilitate the patient's experience of a smooth transition to home care. The CTC is an expert in referral process and the EMR system. Is responsible for ensuring the agency achieves its referral and admission goals by developing and maintaining referral relationships.

Education and Experience Required:

• Graduate of an accredited LPN (or RN) Nursing Program
• Current Maryland Nursing Licensure
• Two to three year's work experience in hospital/acute care setting; Home Care experience a plus

Required Knowledge, Skills, Abilities, and Other Characteristics:

• Proficient Microsoft Office skills, Internet proficiency
• Ability to work independently and multi-task
• Strong telephone customer service skills
• Strong organizational skills
• Ability to coordinate with other teams and managers

Principle Duties and Responsibilities including Direct Reports:

1. Develop and orient new care transition coordinators to entire intake process.
2. Receives all incoming calls from clinical, staff, physicians, and other referral sources.
3. Completes accurate and timely clinical evaluations of potential patients and appropriateness for Home Care.
4. Identifies appropriate attending physician for each patient coming on service and obtains physician orders.
5. Completes accurate and timely referrals inputting clinical and non-clinical documentation into the EMR to facilitate achieving established referral goals.
6. Educate and build relationships with referral sources, patient families, and physicians on Home Care Services, insurance criteria and qualifications.
7. Follow-up on Hospitalized patients from their designated facilities and enter hospitalized follow-up note in EMR system.
8. Obtain all necessary documents for Face-to-Face to ensure timely billing of our Home Care services.
9. Ensures new referral insurance has been verified and appropriate authorization has been provided by insurance coordinator.
10. Coordinates with clinical team for safe patient care.
11. Responds to all requests, questions, and concerns by communicating with respect and courtesy in accordance with AHC RISES to referral sources, physicians, patients, families, and internal/external customers.
