Job Announcement

As a member of an interdisciplinary team that conducts Medicaid screenings and assessments for home-based care services, provides case management services for older adults and adults with disabilities in their homes. This position does not provide direct patient care, but rather uses assessment skills and knowledge of medications and medical conditions in a community setting. Case management services include comprehensive assessments using Virginia's Uniform Assessment Instrument (UAI), care plan development, case coordination, and crisis intervention. Emphasis of assessment is on the client's health status and health care needs are the primary consideration. Assesses functional eligibility for Medicaid Long Term Care Services, including nursing facility, community based waivers and Adult Day Health care. Serves as a resource to families and other service providers regarding services for adults in Fairfax County. In collaboration with the client, develops a client centered, solution focused care plan to meet psychosocial, health, emotional and financial needs. Care plan development may include obtaining input from multidisciplinary teams. Assesses risks posed to clients in accordance with state and local policies and with consideration of client's self-determination and right to least restrictive alternatives. Helps clients and families with plans to eliminate or minimize risk.

This is an opportunity, as a Public Health Nurse, to take advantage of trainings in the Health Department and to meet monthly with the Health Department Long Term Care nursing staff. It is an excellent opportunity to learn about Medicaid and other community services.

Note: This is a field position with much of the time spent conducting assessments in the client's home. County cars or mileage reimbursement is available.

Employment Standards

MINIMUM QUALIFICATIONS:
Graduation from a college or university accredited by the National League for Nursing or the Commission on Collegiate Nursing Education with a bachelor's degree in nursing; plus one year of work experience in a public health/community health setting; or two years of full-time post-licensure experience as a nurse in an equivalent health care setting whose focus was primarily prevention services and community education.

CERTIFICATES AND LICENSES REQUIREMENTS:
  • Possession of a current license or a multistate licensure privilege to practice as a Registered Nurse in the Commonwealth of Virginia. Selected positions or area(s) of assignment may require the incumbent to possess a valid motor vehicle driver's license.
  • Valid driver's license.

NECESSARY SPECIAL REQUIREMENTS:
The appointee to this position will be required to complete a criminal background check, a check of the Child Protective Services Registry and a driving record check to the satisfaction of the employer.

PREFERRED QUALIFICATIONS:
  • Experience working with the elderly and adults with disabilities.
  • Excellent verbal/written communication and interpersonal skills.
  • Proven ability to prioritize work and adjust to changing priorities.
  • Strong attention to detail.
  • Ability to work independently and as part of a team.

PHYSICAL REQUIREMENTS:

Ability to conduct visits in client private homes and facilities which may not be handicapped accessible. Ability to communicate clearly verbally and in writing. Ability to understand and use automated technology. All duties may be performed with or without reasonable accommodations.
