Provide Case Management services to teens who are preparing to age out of the foster care system. Purpose: To monitor Participants practice, development and mastery of independent life skills in preparation for emancipation from foster care. Goals: Challengers Participants will be able to function self sufficiently upon discharge from Challengers independent living program Participants will acquire a skill set that will enable them to function at a high level in day to day life activities. Daily Responsibilities: Track Participants' progress through the status of function program. Track Participants' educational progress. Assign practical concrete tasks for each Participant relevant to his status of functioning. Track Participants' progress in establishing treatment goals/plan. Provide creative interventions to ensure achievement of Participants Educational and Independent life goals. Maintain daily contact notes. Review apartment check form and daily log. Follow-up on fines for the previous night with Participants. Conduct morning apartment checks. Weekly Responsibilities: Attend Clinical Supervision meetings. Update Participant's concrete task checklist. Schedule Face-to-Face contact with Participants to discuss ILS, Home Life and Educational progress Tasks: Update roster of current Participants. Update Participant's status in the Status of Function Program. Continuously develop partnerships with DSS workers for referrals. Submit monthly progress reports to the CEO. Submit ILS curriculum. Develop community relationships with professionals who can be resources for Challengers' Participants. Utilize resources at Challengers to assist Participants in achieving their goals. Develop training activities to present in bi-weekly staff meetings. Qualifications: Masters or Bachelor's Degree in Social Work credentials and credentialed with the following licenses; LCSW-C, LMSW or LCSWA. Reporting: All Case Managers report to the Director of Clinical Services.
