Coordinates all systems and services required to provide patients and families social support and coordinates the discharge plan to assure quality, cost-effective care and efficient utilization of resources across the healthcare continuum. Patient served include neonate through adult-geriatric. *Knowledge of regulatory social/welfare programs including medicine, Medicaid, social security and disability is required. *Knowledge of regulations and legal parameters governing Maryland nursing homes required. *Knowledge and skills to provide social counseling appropriate to the age of the patients served. *Must know knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's requirements relative to his or her age-specific needs. Education & Experience: Social Worker I: *Bachelor's degree in Social Work required. *Two years of social work experience in a hospital setting or social agency preferred. *Prior acute care experience preferred. Social Worker II *Master's degree in Social Work required. *Two years of social work experience in a hospital setting or social agency required. *Prior acute care experience preferred. Licensure & Certification: *Current licensure as a Social Worker in the state of Maryland required. Physical Demands: While performing this job, the individual will work in confined areas and/or the nursing units. The job requires sitting approximately 70% of the time with walking and standing approximately 30% of the time. The job requires visual and auditory acuity and ability to orally communicate. There is a potential for exposure to communicable diseases and/or blood and body fluid substances and hazardous materials.
