Job Details

Purpose of Position - Essential Functions (the fundamental, crucial job duties performed in this position):

The Visiting Med Surg RN is responsible for administering skilled nursing care to patients on an intermittent basis in the home, providing care management, supervising the care provided by the home health aide and LPN, working collaboratively with the nursing supervisor to coordinate Physical Therapy, Speech Therapy, Occupational Therapy, Social Services, Nutritional and Chaplain Services according to physician's orders and under the direction of the nursing supervisor.

Actions and attitude that contributes to the AHC pillars of excellence (People, Quality & Safety, Patient Experience, Finance, Growth, and Population Health Management) and expectations are achieved.

Principle Duties and Responsibilities including Direct Reports:
1. Supervision of field staff Home Health Aid/LPN staff
2. Regularly attends mandatory and non-mandatory meetings
3. Documents patient care and related information according to Federal and State regulations, completed per standards set by Agency
4. Planning and evaluation of patient care that is comprehensive and organized to achieve its goals
5. Meets standards set by agency demonstrating flexibility, commitment and adaptability
6. Appropriately assesses all factors that impact patient and patient's condition
7. Makes appropriate interventions to provide for safe and effective care addressing all of patient's needs

Required Knowledge, Skills, Abilities, and Other Characteristics:
• Ability to access patient needs
• Recommend appropriate care
• Ability to create applicable care plans
• Ability to demonstrates excellent communication and patient experience skills
• Ability to understand and document using Electronic Medical Records (EMR)
• Ability to manage flexible work schedule

Education and Experience Required:
• Associates degree in nursing required
• Bachelors in Nursing (BSN) preferred
• Valid Maryland RN licensure
• Two years full time experience in Medical/Surgical or facility nursing within the past five years
• Valid American Heart Association BLS/CPR Certification
• Valid Driver's License with agency insurance requirements

Description of Working Conditions:
• Occasional strenuous lifting
• Occasional bending, stooping, reaching, pushing, pulling and walking
• Requires flexible work schedule, as workload varies both in time of day that is available to be done as well as amount of work
• Occasionally an extremely stressful environment characterized by multiple requests and changing priorities
• Subject to all weather conditions
• Routine exposure to body fluids
• Potential exposure to infectious diseases, blood and body fluids while performing job related duties
• Hours may be long and unpredictable; evening and weekend hours as necessary

Work Schedule:

Coverage Sunday- Saturday from 5 PM - 8 AM to make after hours emergent visits in Prince George's, Charles , and St. Mary's c
