Summary

The incumbent serves as a GS 11 social worker for the Mental Health (MH) Substance Abuse Treatment Program (SATP) at the Hunter Holmes McGuire VA Medical Center. SATP provides psychosocial Substance Use Disorders (SUD) assessments as part of the consult process when the Veteran's is initially referred to the SATP Clinic and utilizes Evidenced Based Practices (EBPs) for group and individual short-term therapy to patients with a variety of SUD and co-occurring MH diagnoses.

Responsibilities

  • Provide individualize care coordination/psychotherapy service and discharge/aftercare planning highlighting patients' strengths, needs, abilities, and preferences within patient interaction, including family and collateral contacts with appropriate release of information in place.
  • Adhere to Medical Center Memorandums, (MCM) Standard Operating Procedures (SOP) for documentation. Adhere to approved templates and required standards for documentation as outlined by accrediting bodies such as Joint Commission, CARF, and/or revenue sources providing funding to VHA.
  • Provide documentation sufficient to meet the needs of various interdisciplinary teams engaged in treatment/aftercare planning process both within the facility and for potential placement and/or aftercare programs/facilities.
  • Adhere to MCM, SOP, and directives for clinical contact with patients, families, and collateral providers over the course of treatment, which includes screening/intake, program participation, and post discharge follow up/aftercare.
  • The incumbent will participate in ensuring patients have access to care via completion of required documents, 1010s, Advanced Directive, Consults (per credentialing/policy) Care Coordination Notes (name may vary) and other forms that assist with access to care within the VA or the community.
  • The incumbent will participate in survey accreditation(s) ensuring standards are adhered to within their social work practice, unit/service and at the facility level as well (i.e. infection control and safety).
  • The incumbent will participate in continuous survey readiness through ensuring data for PI projects, and other documentation required for accreditation are current/up to date.
  • The incumbent will participate in all assigned meetings to include individual, team (huddles, monthly) service (both social work and matrix) staff meetings.
  • The incumbent will participate in social work on call rotation to accommodate patient care on weekend, holidays, evenings, and nights.
  • The incumbent will participate in coverage for social work services on the assigned unit, and when needed for other social work teams' inpatient and outpatient as needed.
Work schedule: Monday - Friday 8:00 AM to 4:30 PM with occasional weekend coverage

Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 06/20/2019.

Basic Requirements:
    United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.
Loss of Licensure or Certification: Once licensed or certified, social workers must maintain a full, valid and unrestricted independent license or certification to remain qualified for employment. Loss of licensure or certification will result in removal from the GS-185 social work series and may result in termination of employment.

Preferred Experience: Skill in assessing and treating the complicated psychosocial problems of Veterans referred to the SATP Clinic.

Grade Determinations: GS-11 Social Worker

Experience, Education and Licensure: Promotion to the GS-11 full performance level requires completion of a minimum of 1 year of post-MSW degree experience in the field of health care social work (VA or non-VA experience) and licensure or certification in a state at the independent practice level.OR,In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting. In addition, the candidate must demonstrate the KSAs in subparagraph below.

Demonstrated Knowledge, Skills, and Abilities
1. Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services
.2. Ability to independently assess the psychosocial functioning and needs of patients and their family members and to formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team.
3. Ability to independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. This requires knowledge of human development and behavior (physical and psychological) and the differential influences of the environment, society and culture.
4. Knowledge and experience in the use of medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.
5. Knowledge of psychosocial treatment and ability to independently implement treatment modalities in working with individuals, families and groups who are experiencing a variety of psychiatric, medical and social problems to achieve treatment goals. This requires independent judgment and skill in utilizing supportive, problem solving or crisis intervention techniques.
6. Ability to independently provide counseling and/or psychotherapy services to individuals, groups and families. Social workers must practice within the bounds of their license or certification. For example, some states may require social workers providing psychotherapy to have a clinical level of licensure.
7. Ability to provide consultation services to other staff about the psychosocial needs of patients and theimpact of psychosocial problems on health care and compliance with treatment. Ability to provide orientation and coaching to new social workers and social work graduate students. Ability to serve as a field instructor for social work graduate students who are completing VHA field placements.
8. Ability to independently evaluate his/her own practice through participation in professional peer review case conferences, research studies, or other organized means.
9. Knowledge and skill in the use of computer software applications for drafting documents, data management, and tracking, especially those programs in use by VHA.

Physical Requirements: The work is primarily non-strenuous though a significant amount of walking is required for face to face contact with patients, family members and medical staff. Ability to life 5- 10 pounds, mostly documents and/or binders is required. The ability to sit for 1-2 hours, and do substantial computer work.
