The VA Maryland Health Care System (VAMHCS) is a Level I tertiary multi campus teaching facility affiliated with the University of Maryland, School of Medicine and over 100 educational institutions. The VAMHCS consists of the acute care Baltimore facility; the multi building Perry Point campus with acute medicine, mental health and long term care; the Loch Raven campus with long term care; and six urban and rural outpatient clinics.

Responsibilities

The Patient Aligned Care Team Nurse Coordinator is a self-directed professional registered nurse (RN) who assumes leadership role in direct and indirect care management of a group of patients through the coordination of care with physicians, nurse practitioners, medical residents/students, other health care professionals and hospital systems. The incumbent is accountable for adhering to established policies, standards of care and standards of practice set forth by the Joint Commission. The RN will assume care coordinating responsibility for the patients assigned to them in the management of chronic and acutely ill patients. The incumbent will work with the patient care team to guide, teach and implement patient care plans.

The RN works with the team to ensure that the needs of the patient are met in a timely manner and in a customer focused environment. In addition to face- to- face communication, the RN will communicate telephonically and return phones calls, and take messages in collaboration with the team members to ensure that all patients' management/care coordination issues are addressed in a timely manner. The incumbent will assist patients in preparing for upcoming appointments and ensure patients are informed.

The RN will be responsible for providing intensive management and coordination of care for patients with acute and chronic disease states in Primary Care environment. The RN will provide the Nursing Leadership and guidance for other team members in providing direct and indirect care for an assigned group of patients. The RN along with the provider will be responsible for ensuring that assigned patients are meeting the prescribed clinical goals to include Ambulatory Care sensitive indicators, HEDIS, and applicable ORHS measures. The RN will be responsible for using computer applications to monitor and report clinical data.

Work Schedule: 8 hour shifts between the hours of 7:30am-5:30pm, Monday-Friday. Occasional Saturdays 8:00am-12:00pm. No holidays.

Qualifications

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing ( ACEN ) or The Commission on Collegiate Nursing Education ( CCNE ). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
  • Proficient in speaking, reading, and understanding the English language.
  • Physically, mentally and emotionally fit to perform the duties of the position.
Preferred Experience:
  • Bachelor of Science in Nursing (BSN) is preferred.
  • Primary Care/ Ambulatory Care nursing practice 2-3 years, within the last 5 years preferred.
  • Current BLS certification required

Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
  • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
  • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
  • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
  • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree.

Physical Requirements: It is VA policy to ensure that all selected applicants and employed personnel are physically, mentally and emotionally fit to perform the duties of the position to which assigned. A pre-placement physical examination is required for all full-time, part-time and intermittent employees identified by VA Directive and Handbook 5019. The pre-placement physical examination shall be completed prior to appointment to determine the physical and mental fitness for candidates for appointment in VA. The examination will be concerned with the mental and physical ability of the applicant to satisfactorily perform the duties of the proposed assignment and will include requisite laboratory and other screening as required by Federal regulatory agencies.

Additional information

Annual leave shall accrue for full-time Nurses, and Nurse Anesthetists at the rate of 8 hours for each full biweekly pay period.
